Nemours is seeking a Community Health Worker to support our Seaford, DE location. Position is 40 hours per week.

The Community Health Worker (CHW) will be responsible for helping Nemours Children's Health patients and their families to navigate and access community services and other resources and adopt healthy behaviors. The CHW supports providers and the Care Management team through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their families, provide social support and informal counseling and advocate for individuals and community health needs. The CHW will promote the health and well-being of patients and families by providing the re-education of and coaching to families on disease/health management.

This position is responsible for: advocating, facilitating and organizing access to health and social services with/for an identified community to improve the health and well-being of community members. Community outreach, such as calls to the home, home visits and health screenings will be required. This position will be in the community and visiting families in their homes and provider offices. As a priority, CHW activity acts to promote, maintain, and improve the health of patients and their families, provide social support and informal counseling and advocate for individuals and community health needs.

Position is full-time, 40 hours per week.

Responsibilities:

  • Establish trusting relationships with patients and their families while providing general support, encouragement and promoting general health and well-being.
  • Advocating, facilitating, and organizing access to health and social services with/for families to improve health and well-being of the community at large, particularly those families who have children diagnosed with a chronic disease.
  • Provide case management of non-medical needs to a caseload of high-needs families through regular and ongoing communication ensuring engagement of and a collaborative approach to working with them through an integrated approach to care management and community outreach.
  • Utilize EMR to document patient care, communicate with the primary care team, and collect data at set intervals through assessment tools, collaborating with the leadership to reflect issues that affect health and gauge patient progress.
  • Conduct intake interviews with patients/families, including enrolling and/or referring patients into appropriate community programs. Provide referrals for services to community agencies as appropriate. Identify, create, and nurture relationships with local agencies, schools, churches and other programs that can provide services to children and their families.
  • Provide health re-education on topics related to medications, therapies, health promotion, and informal counseling. Accomplished through partnership with patients' medical team, social work, and care coordinators for follow up with patients/parents via phone calls, home visits and visits to other settings where the patient can be found.
  • Assist in managing care and care transitions for vulnerable populations by establishing supportive relationships with families via regular and ongoing communication to identify and address issues that may contribute to poor health and readmissions. Help patients connect with transportation resources and give appointment reminders as appropriate.
  • Collaborate with the medical and social work team to address and document non-medical issues that affect the health of patients. This includes all social determinants of health needs including but not limited to food insecurity, financial hardships, transportation, housing, violence prevention, etc.
  • Utilize problem solving skills to assist in unusual or difficult patient/family situations. Assist with access to medical insurance, specialty pharmacies.

  • Qualifications:
    • Minimum of one year of prior experience as a Community Health Worker or related experience providing education to individuals on health management.
    • High school diploma required

    • Attend CHW Core Competency training program
    • Become certified as an asthma educator within 3 months of hire
    • Attend and complete motivational interviewing training
    • Attend basic skills for working with smokers training

    NYCP is seeking a Public Health Data Analyst to join our Live Healthy! team. The Public Data Analyst will work closely with the Director of Nutrition, providing oversight and quality assurance of the Live Healthy! data; creating effective tools for both gathering and organizing the data; providing accurate data analysis; collaborating with the Program Director on reporting documents; and the daily function and coordination of NY Common Pantry's office/administrative systems; Performing in-depth statistical analysis with presentation of findings; Supporting data interns with semester placements and project development. As a part of the NYCP Bronx office, the Data Analyst will support the Live Healthy! Program and NYCP as a whole to assess the impact and effectiveness of nutrition education programming. A strong emphasis is placed on providing evaluation and quality assurance of the SNAP-Ed program to show its effectiveness and efficacy.

    The Public Health Data Analyst will report to the Director of Nutrition Education at the 788 Southern Boulevard, Bronx, NY 10455 location. The position is a full-time, exempt position.

    Responsibilities:

    • Acquire data from primary or secondary data sources and maintain databases/data systems.
    • Work closely with management to complete program reports (monthly, quarterly, and annual reports).
    • Manage and oversee data interns from stakeholder institutions for community nutrition rotations, as needed.
    • Implement and create processes that improve survey data processing and significantly reduce and/or eliminate manual data entry.
    • Develop complex data systems and procedures that connect different data sources using Visual Basic and Power Queries in Microsoft Excel and Microsoft SQL in Access that streamline data processing.
    • Design and implement efficient and effective procedures for data management and analysis with attention to all technical aspects.
    • Provide technical assistance and training to team members and staff.
    • Analyze and interpret data, both from quantitative and qualitative analyses, using accepted statistical and qualitative research methods.
    • Perform independent research to identify and include new demographic, health, and food systems data sources for needs evaluations and comparative analysis.
    • Present findings from survey data to management and the team, and collaborate with staff to improve survey tools and reporting processes.
    • Participate in leadership meetings with the Manager and Program Director to help develop a future vision and objectives for the Live Healthy! program, take on leadership roles in project planning, troubleshooting, and implementation of new strategies and initiatives.
    • Use mapping software such as ArcGIS to evaluate needs and visualize program results.
    • Explore and implement new and innovative ways and technology to evaluate program objectives and present program successes and outcomes.
    • Examine large-scale and long-term datasets to evaluate overall program impact and reach.
    • Support other Food Program departments with data analysis efforts.

    Qualifications:

    The Community Health and Wellness Department at LaGuardia Community College offers four undergraduate degree programs: Human Services: Mental Health, Nutrition and Culinary Management, Public and Community Health, and Therapeutic Recreation.     Job Qualification
