Qualifications:

Advocate & Support. As a Licensed Clinical Social Worker you will assess, educate and collaborate with patients on care planning that addresses everything from work, travel and financial concerns to emotional support. Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. + Conduct in-center and face to face visits as necessary to assess patient needs and implement a plan of care (up to 50% same day travel with this role).
+ Educate patients on their insurance options including Group Health Plans (GHPs), Medicare, Medicaid, and managed care plans. Serve as a liaison between patients, insurance companies, and the facility staff
+ Assists patients in understanding their financial responsibilities, payment arrangements, and potential financial assistance programs
+ Maintain accurate records of patient insurance information, care plans, and assessments
+ Perform other duties as assigned

Qualifications
+ Master's degree in social work.

+ Dialysis experience in a renal setting working with ESRD & CKD patients
+ Excellent communication skills; ability to communicate clearly and concisely both orally and in writing to patients, family members and various professionals.
+ Basic computer skills and understanding.
Preferred
+ Five (5) years of related work experience.
+ Two (2) years of experience working with the dialysis/kidney care population.
+ Fluency in both English and Spanish
+ Track record of excellent customer service skills.
+ Medicare or Medicaid experience preferred
Travel
Travel between outpatient facilities is required.

The Social Worker in the Mother Baby Unit provides compassionate, patient-centered psychosocial support to new mothers and their families. This role plays a key part in ensuring the emotional well-being of patients while facilitating safe and supportive discharge planning.
**What You Will Do:**
+ Conduct comprehensive psychosocial assessments for postpartum patients and families
+ Provide emotional support and counseling for issues such as postpartum challenges, anxiety, and stress
+ Collaborate with physicians, nurses, case managers, and other members of the care team to ensure holistic care
+ Develop and implement discharge plans with appropriate community referrals and resources
+ Educate patients and families on parenting resources and coping strategies
