ChristianaCare HomeHealth is seeking a Full Time LPN . The successful candidate will support the New Castle, DE and surrounding area; work day shift hours, one weekend a month, 2 holidays a year, and two on-call shifts a month.
Duties and Responsibilities:
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Must be IV proficient, upon hire we will train new staff and expect proficiency at 90 days.
- Develop and implement care plans, which are goal oriented and patient centered and consistent with available resources.
- Develop plan for discharge of patients.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Current license to practice as an LPN in the State of Delaware
- Associates degree or completed an accredited LPN education program.
- Work experience in an acute or chronic care facility, or home health agency is preferred.
SPECIAL REQUIREMENTS:
- Proof of a valid driver’s license and active auto insurance is required. Alternative transportation arrangements may be considered based on operational need.
