- Must be able to lift, move or assist patients using proper body mechanics. The ability to occasionally lift or push up to 50 lbs. and pull up to 100 lbs. is also required.

Essential Skills
+ Minimum of 1 year experience in a medical/healthcare setting or 2 years in a customer service role.
+ Experience providing a high level of customer service in a fast-paced healthcare environment, including handling confidential data with discretion.
+ Proficiency with Electronic Medical Record systems, including EMR, Epic, and care coordination.
+ Knowledge of medical terminology, CPT, ICD10 coding, insurance authorization, and prior authorization.

+ Coordinates and schedules extensive diagnostic tests necessary for transplant evaluation and coordinates continuum of care for kidney care patients
+ Interacts with physicians (community and UWMC) and nurses to schedule all diagnostic tests as ordered by physicians in a timely manner

+ Interacts with patient to discuss itinerary of the diagnostic tests, pre-test patient preparation instructions and answer questions
+ Orders interpreter and translation of correspondence as needed
+ Holds responsibility for continuum of care steps following initial kidney transplant appointment until listing and annual updates while on kidney waiting list: diagnostic test, procedure and appointment scheduling, outgoing referrals and coordination of care with PCP/nephrologists' offices, and completion of missing outside provider documentation
+ Contacts various departments to schedule program tests; completes necessary requisitions and consults at UWMC and/or outside facilities
+ Prepares schedule for diagnostic tests and clinic, plus corresponds with patients to provide necessary program instructions
+ Coordinates the receipt of requirements: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians' offices, nephrologists' offices and other medical facilities
+ - Receives outside laboratory results on kidney care patients undergoing serial laboratory monitoring per their treatment plan
+ In preparation for return clinic visits, ensures recent lab work, radiology results, and pathology reports are completed and available
+ Follows up to ensure exams are completed or rescheduled if necessary and notifies MD/RN of unusual delays in the completion of exams
+ Assists physicians and nursing staff maintain systems to safely manage patients on the kidney transplant waiting lists, especially updating demographic information and scheduling required tests and appointments

An unlicensed caregiver who maintains a safe and clean environment for patients and Health Care team members. Transport patients with proper technique. Transport equipment, supplies and specimens. Provide environmental sanitation as required.
Essential Responsibilities:
+ Assists in preparation of the unit, patient rooms, and beds for receiving patient admission, returning surgical patients, transfers, emergencies, or discharges. Provides hygiene care, which includes providing or assisting in bed baths, oral hygiene, hair care and shampoo, back care and rubs, perineal care, dressing patient, and changing bed linen.
+ Prepares patients for meals, serves and removes food trays, and assists with meal or feeds patients. Distributes water and nourishment to patients. Performs other duties as assigned by the RN as patient care needs change. Orients patients and significant others to call lights, bed controls, side rail usage, bathrooms phones, TV waiting areas, and visiting hours. Assists and accompanies patient and their belongings (including personal property) in the admission, transfer, and discharge process. Answers patient call lights, anticipates patient care needs, and makes rounds to assigned patients on an ongoing basis. Communicates with patients in a courteous and helpful manner. Establishes and maintains communicates with the health care team. Performs data collection and documentation of TPR, BP, wt, ht, I/O, activity, diet, and hygiene in an ongoing, timely, accurate, and concise manner.
+ Demonstrates the use of good body mechanics and provides for patient safety when transferring, ambulating, turning, lifting, and positioning patients. Acts appropriately in emergency situations. Performs routine treatments and patient care duties listed on the skills inventory form and under the direct supervision of the RN.
+ Identifies and reports changes in the patients condition to the RN within an appropriate time frame. Maintains verbal and written information in a confidential manner. Assists in straightening patient rooms, cleaning overbed tables and bedside stands, keeping patient care areas clean at end of each shift. Collects and transports specimens for delivery to appropriate departments per hospital standards.
+ Completes work order for equipment that is out of service and leaves copy in the appropriate designated place/unit for follow up. Adheres to Infection Control Policy including but not limited to: a) demonstrates body substance precautions by wearing gloves and other protective attire to avoid body substance contact when appropriate; b) adheres to the KFHPW hand washing policy; c) complies with the proper handling of waste; d) maintains a clean work area. Provides postmortem care under the direction of the RN. Demonstrates initiative and responsibility for improving skills and expanding knowledge through continuing education or on the job learning experience. Consistently demonstrates an appearance consistent with current KFHPW dress code policy.
