We seek a **Clinical Pharmacist - Patient Care** for our Pharmacy Value Performance team. This person is responsible for providing ambulatory care clinical pharmacy services including but not limited to the evaluation, interpretation, and management of medications to improve the clinical outcomes and quality of healthcare to patients through the use of value-based therapeutics. The candidate will collaborate with physicians and other healthcare professionals and applies medication management strategies in alignment with evidence-based medicine.

**Primary Responsibilities:**
+ Consistently exhibits behavior and communication skills that demonstrate Optum's commitment to high levels of customer service to internal and external customers
+ Executes disease state management programs targeting chronic conditions such as anticoagulation therapy, heart failure, diabetes, hypertension, COPD, and other diseases
+ Performs post-hospital medication reconciliation, identifying and resolving medication related problems and coordinating patient care post-hospitalization
+ Appropriately manages, assesses and recommends evidence-based medication therapy based upon the patient's clinical needs for optimizing care outcomes and reducing any potential patient safety problems (i.e. drug interactions, suboptimal medication regimens, high risk medications etc.)
+ Analyzes medication history to formulate clinically sound recommendations through medication reconciliation
+ Demonstrates appropriate judgment on conditions requiring consultation, referral or evaluation by patient's PCP or other health care professional
+ Educates patients and / or families about preventative care, medical issues and use of prescribed medications to enhance healthy behavior in alignment with disease management program goals and objectives
+ Addresses new patient referrals, labs, tasks, patient follow-up and drug information questions in a timely manner and consistent with department policy and procedures
+ Fully documents all aspects of patient care and completes all required paperwork for billing and administrative purposes to meet business and compliance requirements
+ Utilizes pharmacy data and other reports to develop and implement strategies to improve quality measures
+ Maintains productivity standards through the ability to multi-task to complete tasks in a thorough and timely fashion
+ Maintains competency by participating in and pursuing professional development activities which align with the expanded services provided by the department (including but not limited to chronic disease management, transitions of care, quality initiatives, cost effective prescribing, and education for clinicians)
+ Assists in clinical instruction and supervision of medical assistants, certified pharmacy technicians, pharmacy students and others as required
+ Uses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
+ Performs additional duties as assigned

**Required Qualifications:**
+ Bachelor's degree in Pharmacy or PharmD
+ Current, unrestricted California State Pharmacist License
+ 3+ years of experience in clinical pharmacy practice including clinical pharmacotherapies, disease state management and drug use evaluations OR completion of 1 or 2 year ASHP-accredited pharmacy residency program in Clinical Pharmacy Practice or equivalent background/experience

**Overview**
The Manager of Clinical Services of the PICU plays a vital leadership role in the oversight of daily operations, clinical excellence, staff development, and the promotion of CHLA's high standards of compassionate, family-centered care. This leader is responsible for setting the tone for a professional practice environment and fostering a culture of safety, collaboration, and evidence-based practice. The Manager ensures optimal patient outcomes through continuous performance and quality improvement, resource management, and regulatory compliance. This role includes direct engagement in strategic planning, team building, and interdisciplinary collaboration. The Manager empowers frontline staff through coaching, education, and delegation, while also ensuring that all practices are developmentally and age-appropriate across the continuum of pediatric critical care.
The PICU at CHLA is a 36-bed, high-acuity unit spanning two hospital locations. Our team provides multidisciplinary, family-centered care to critically ill infants, children, and young adults with complex medical and surgical needs. Our patient population includes:
Post-liver and kidney transplant patients
Level I trauma cases
Patients with rare or complex diagnoses
Advanced respiratory and cardiac support needs

**Minimum Qualifications/Work Experience:** 3-5 years of progressive PICU clinical experience required; Previous leadership or management experience strongly preferred; Proven success in team leadership, problem-solving, and quality improvement initiatives; Strong interpersonal, verbal, and written communication skills; Effective team builder and motivator; Highly organized and adept at managing multiple priorities; Demonstrated ability to lead through change and inspire clinical excellence

Job Summary:
Serves as the Clinical Manager responsible for supervising the activities of the home/health/hospice agency. Monitors quality, service and utilization standards. Supervises the day-to-day clinical operational activities. Ensures compliance with federal, state, and local regulations.
Essential Responsibilities:
+ Manages the day-to-day clinical operations of the agency, including practice standards, staffing, payroll, budgets, fiscal management, and quality improvement.
+ Supervises agency and employee safety programs and risk management.
+ Responsible for overall supervision of hospice and home health nurses and aides, includes interviewing, hiring, monitoring assignments and evaluating personnel.
+ Develops and implements action plans to improve staff development.
+ Acts as resource to staff as clinical expert.
+ Ensures individualized care goals are met.
+ Ensures the highest quality of care is provided and is in compliance with federal, state, and local regulatory requirements and established departmental policies and procedures.

We are seeking a highly skilled and dynamic Director of
Clinical Services to lead the development, evaluation, and implementation of
multiple clinical programs and medical services with an emphasis on optimizing
Utilization Management for the Faculty Practice Group (FPG) Management Services
Organization (MSO). Reporting to Chief
Medical Officer of Faculty Practice Group and Medical Group, this role is pivotal in ensuring the
delivery of program functionalities in all aspects of Utilization Management
