Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. **DUTIES / ACTIVITIES:**
CUSTOMER SERVICE:
+ Responsible for driving the FMS culture through values and customer service standards.

PATIENT CARE:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
+ Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
+ Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
+ Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
+ Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor.
+ Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
+ Perform administration of Heparin as delegated or as allowed by state law.
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures,
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
+ Perform and record Pre and Post dialysis evaluation, weight and vital signs with initial identification.
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the nurse supervisor.
+ Observe patient, and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the nurse supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
+ Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application accurately and in a timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure omitted entries are completed or corrected where appropriate.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
OTHER:

**Job Summary:**
A Patient Care Technician (PCT) provides basic health care and support to patients under the guidance of the Registered Nurse or designated healthcare professional. As part of our team, the Patient Care Tech (PCT) is responsible for sanitizing and cleaning patients' rooms, monitoring patients' health and vital signs as well as assisting patients with basic needs like feeding and grooming.
**Essential Job Functions**
+ Provides basic patient care to include, but not limited to, care and comfort, vital sign measurement, personal care and hygiene, and assists with mobility, including unit-based specialty duties.
+ Acts as liaison between patient and nurse to report changes and/or concerns.
+ Provides high-level customer service to all patients, patient's family, visitors, and employees.
+ Provides accurate, precise, timely documentation when applicable for patient care.
+ Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving.
+ Performs delegated tasks once competency has been validated.
+ Mobilizes patients using therapeutic transfer techniques which foster independence and safety.
+ Fosters independence by encouraging patients to complete tasks as independently as possible.
+ Individualizes care based on patient's interests and hobbies.
**Education:**
+ None
**Licensure/Certification:**
+ BLS Basic Life Support (preferred upon hire, must obtain from approved source prior to direct patient care)
+ Must have completed one of the following: (required)Successful completion of nursing aide training program ORSuccessful passing of STNA State Tested Nurse Aide (required if OH LTC), OR CNA Certified Nurse Aide, OR Nurses Aide state testing or completed a nursing program that had one clinical nursing course and one clinical rotation OR 1 year of relevant experience in a clinical setting
**Experience:**
+ Recent experience in acute care, physician's office, home health/hospice, or long-term care facility, with a preference for the setting in which they are applying (preferred, not required)
**Skills & Abilities:**
+ Possesses problem-solving skills with excellent communication and interpersonal skills
+ Engage with staff and patients in a professional manner
+ Ability to be proactive in a rapidly changing environment
**Training**
EPIC Electronic Health Record (EHR) training (preferred)

MINIMUM QUALIFICATIONS
High School Diploma or Equivalent AND one of the following:
· Completion of a High School Medical Assistant Career Technical Program, or
· Graduation from an accredited School of Medical Assisting, or
· Completion of Military Medic training, or
· Certification as a Paramedic (EMT-P), or
· At least one year of experience as a physician-trained Medical Assistant in a similar clinical setting.
This position is responsible for administrative and clinical duties delegated by the physician. The Medical Assistant is responsible for assisting physician with patient care in compliance with the Ohio Administrative Code. Other responsibilities include, but are not limited to, routine clinical and administrative duties as needed to meet the needs of the practice to which it is assigned. The duties of the Medical Assistant vary from office to office based on size and specialty of the practice.
**Responsibilities And Duties:**
Facilitates efficient and effective patient flow including preparing the patient for the visit and providing specific services as determined by providers in accordance with provider licensure, OH policy and scope of practice.
Successful completion of Care Connect EMR documentation skills.
Performs necessary administrative duties that assure all information is current and updated. Includes maintenance and monitoring of EMR in basket.
Maintains and cleans environment, work area, equipment, and communicates any equipment malfunctions to appropriate dept/unit personnel.
Communicates with patients and customers in a respectful and caring manner.
Communicates, collaborates, and anticipates the needs of the health care team to ensure continuity and quality of care and coordination of services.
Collects and monitors patient data; reports patient data and patient need as directed.
Maintains documentation guidelines around patient chargeable items/services.
Ensures processes and services are continuously monitored for quality, cost effectiveness, and efficiency.
Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.
Engages in continuous study of the entire professional field, including best practices, to maintain the professional competence, knowledge, and skills necessary for the satisfactory performance of all assigned responsibilities.
Completes required continuous training and education, including department specific requirements.
Promotes and advocates for patient quality, safety, and experience initiatives. Acts as a role model.
Serves as SME/resource to the dept/unit/practice in areas such as quality, performance improvement, and EMR (i.e., EPIC Super User).
Has oversight of efficient and effective patient flow including preparing the patient for the visit/admission and providing specific services and education as determined by the providers in accordance with licensure and policy.
Leads and supports development of standard work and develops necessary customizations.
Functions as a clinical preceptor and is responsible for training new staff.
Is compliant with scope of practice.
**Minimum Qualifications:**
