Home Care Licensed Practical Nurse (LPN): Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN would provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Hours: Full-time | Days | Primary hours would be Monday - Friday from 8:00am - 5:00pm | Weekend and Holiday Rotation is dependent upon staffing levels. Work approximately two Holiday's per year and one weekend every 4 - 6 weeks.

Territory: This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position of where the patients live would be in the Southeast Metro (the cities that are south of highway 94 and east of 35W).
A few must-haves for this position:
- Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
- 2 years experience as an LPN.
- Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance is required.
- CPR Certification

- Completes timely and accurate clinical notes including addressing patient’s progress.
