We are seeking an innovative and energetic Public Health Nurse to join our clinical team at the forefront of public health, providing services to patients in our medical clinics as well as reaching out to the community. Our team provides excellent, holistic medical care in the areas of women's health, cancer prevention, maternity care, family planning, immunizations, and STD/STI.

The Public Health Nurse will work both independently and as an integral member of a district team, providing professional nursing care to patients in clinics and occasionally in community settings. Specific duties include conducting nurse assessments, taking vital sign measurements, drawing blood, administering vaccines and medications, performing and interpreting lab tests, developing and implementing care plans, and providing case management. As a member of the district clinical team, the Public Health Nurse will participate in emergency drills and respond to real medical emergencies; will be expected to participate in public health disaster exercises and may be requested to serve on an Incident Management Team during actual public health emergencies; and may be requested to help support community health assessments, community health improvement plans, and community outreach events.

This position requires licensure as a Registered Nurse in the Commonwealth of Virginia, and the incumbent must abide by all relevant state codes, regulations, and HIPAA laws. Some travel will be required within the health district and may occasionally be required outside of district boundaries, with mileage reimbursement provided for any travel conducted with a personally owned vehicle. Typical work hours are Monday through Friday, 8:00 am to 4:30 pm.

Job Duties

The Chesapeake Health Department is seeking a resilient Public Health Nurse with a drive to promote health, preventing disease and support the community. The Long-Term Support Services (LTSS) is a Department of Medical Assistance Services (DMAS) mandated program. The Registered Nurse (RN) / Public Health Nurse (PHN) works in partnership with a Social Worker (SW), from the local Department of Social Services (DSS), to conduct joint home visits for adults that are imminent risk for nursing facility placement. Both the RN and SW travels throughout the City of Chesapeake to complete each screening. The RN / PHN will also be responsible to provide screenings for pediatrics, this visit is done without a SW. The PHN will perform other duties to support the district; other services include immunization, emergency preparedness and response (EP&R) and communicable disease (CD) responses. The ideal candidate will be experienced in performing nursing assessments across the lifespan in both clinical and community settings. This position provides additional experiences with case management, resource referrals, computerized charting, EMR, and proficient use of Microsoft applications as necessary. Previous home care visiting experience is a plus.

Personal vehicle travel is required for this position.
This position is for 40 hours per week Monday - Friday 8:15 am - 5 pm, including 45 minutes for lunch.

Please note in the event of a local public health emergency and / or an emergency declaration by the Governor, this position may be required to assume other duties for the districts' emergency preparedness and response (EP&R) activities. During declared emergencies, this will require an adjustment to work hours, location, job duties as well as participation in trainings to meet public health business needs. Additional potential work-schedule variations including participation in community outreach events and other EP&R assignments, such as communicable disease and epidemiological investigation events.

Minimum Qualifications

The Position

  • Providing Patient Triage: the safe, effective and appropriate disposition of health related problems

  • Checks supplies for nursing patient encounters and notifies manager or supply ordering designee if additional stock needed
  • Administers all medical tests and procedures as per protocol
  • Assists with preventative medicine efforts (e.g.: flu, STD, vital sign checks, occupational health)

Project Coordinator (IISAR) Responsibilities Include:

  • Coordinate the IISAR data collection process, including scheduling, stakeholder communication, and reporting.

  • Facilitate communication between the CDC and external stakeholders regarding IISAR submissions and progress.
