**Education**
+ Bachelor's Degree in Nursing or health related field
+ Current and unrestricted Registered Nurse licensure in Pennsylvania
**Experience**
+ 3+ years of clinical experience at the bedside (as a Registered Nurse) working with medically fragile pediatric patients in an acute care, home care, or special needs clinic
+ 3+ years of Case Management experience, preferably telephonic within a managed care organization, desired
+ Eligible to sit for Case Management certification after 12 months of employment. Must obtain this certification after 2 years of employment
+ Demonstrated proficiency working within a Windows 11 environment to include MS Office (Excel, Word, Outlook) and electronic medical record and documentation programs

**Position Description:**
Responsible for providing and documenting individualized, person-centered psychosocial assessment, advance care planning, complex discharge planning, and ongoing emotional support for individuals with serious illnesses and their families within an acute care hospital setting. As part of an interdisciplinary consultative palliative care team, the social worker collaborates with members of their own team plus interdisciplinary members of various other medical teams caring for the patient during their hospitalization. The palliative care social worker helps to ensure continuity of care and a medical care plan and discharge plan that are both in alignment with the patient and family goals of care. Responsibilities
The Assistant Director of PSSM supports the Director of PSSM with planning, organizing, and evaluating all social service programs in Philadelphia. They are responsible for overseeing the Director of Emergency Housing, the Pathway of Hope program in Philadelphia, supporting Philadelphia Corps case managers to enhance services across Philadelphia, evaluation of PSSM housing programs and social services to ensure compliance with funders and meeting the needs of community, and identifying service areas needs and developing strategies to address them. Qualifications
MSW plus 2 years’ administrative experience in Social Work setting providing program and financial oversite required. LSW preferred. Knowledgeable of federal, state, and local guidelines relating to subsidized housing for elderly, disabled and homeless populations. Experience with individuals and families who have experienced homelessness. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check Must have an understanding of and support the Mission of The Salvation Army and the Sanctuary Model.

+ Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
+ Experience in a long-term care facility or with a community-based geriatric program preferred.
+ Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
+ Ability to work with minimal supervision and exercise independent judgment.
+ Superior written and verbal communication and presentation skills appropriate for audience comprehension.
+ Valid driver's license and use of personal vehicle for home care visits
+ Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.
+ Position may require 10% travel within the Centers geographic region and use of personal vehicle.
