Qualifications:

**Job Summary and Qualifications**
The Social Worker (BSW) is responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the BSW acts as a liaison between the patient and community resources/post-acute care providers. The BSW creates and facilitates the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The BSW facilitates patient movement across the continuum of care. The scope of this role includes primarily Med/Surg units.
What you will do in this role:
+ You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
+ You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.
+ You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
+ You will obtain input from the RN case manager to ensure the discharge plan is appropriate and aligns with the patient care plan. You will reassess and modify the patient discharge plan as needed, in conjunction with the clinical team.
+ You will participate in the interdisciplinary care planning process and perform proactive discharge planning.
+ You will provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
+ You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.

This role requires a deep understanding of how socio-economic, behavioral, and environmental factors impact a patient's ability to engage in healthcare and influence health outcomes. Ideal experience includes prior work in primary care or working as part of an interdisciplinary team-particularly in partnership with primary care teams, community health workers, pharmacists, and behavioral health specialists.
**Duties and Responsibilities**
+ Conducts comprehensive assessment of behavioral health needs and formulates diagnosis, treatment plan, goals for next steps for patient care
+ Employs a variety of strategies, approaches, and modalities to address acute behavioral health symptoms, with a focus on CBT, motivational interviewing, solution-focused, and mindfulness techniques
+ Identifies barriers that hinder effective engagement
+ Focuses on patients' high-risk symptoms or frequent hospital utilizers to identify ways to extend wellness and decrease utilization.
+ Ensures patient progression towards desired outcomes by monitoring symptom progression through scheduled visits
+ LCSW offers recommendations to providers about external behavioral health resources, SDOH needs, and other community partners which may support patient needs.
+ Participates in care planning process within multidisciplinary team to collaboratively address patient needs
+ Attends high-risk rounds as an active participant, making BH recommendations and supporting high risk patients.

**Job Summary and Qualifications**
Responsible for the assignment of staff, and coordination of day-to-day workflow and departmental operations.
What you will do in this role:
+ Demonstrates responsibility and accountability in ensuring all admissions and continued stays meet clinical criteria for appropriateness and medical necessity. Assesses discharge planning needs and coordinates the delivery of care to meet these needs.
+ Develops, initiates, and coordinates individual patient case management plans to ensure appropriate level of care across the continuum. Ensures plan meets patient's clinical, psychological and discharge needs in collaboration with the attending physician and interdisciplinary team.
+ Assesses the health status of assigned patients by collecting and analyzing patient and family data, and expedites and coordinates the delivery of services to facilitate patient's progression through the healthcare system.
+ Effectively initiates the case management process to ensure that patients receive the appropriate level of care across the continuum, cost effective medically necessary treatment timely, appropriate discharge planning.
+ Performs admission certification screening for patients in accordance with Utilization Plan using ISD criteria, and performs concurrent stay reviews at least every 72 hours for medical necessity using ISD criteria. Maintains documentation on discharge planning notes and charts on patient's record.
+ Evaluates patients' educational needs concerning continuum of care and available services and provides information and materials regarding community resources, discharge options, and pre and post treatment options and costs to patient and patient families.
**What qualifications you will need:**
+ Registered Nurse, or Advance Practice Registered Nurse (APRN)
+ Registered Nurse Diploma, or Associate Degree

**Responsibilities:**
+ Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
+ Utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.
+ Conducts comprehensive evaluation of Members using care management tools and information/data review
+ Coordinates and implements assigned care plan activities and monitors care plan progress
+ Conducts multidisciplinary review to achieve optimal outcomes
+ Identifies and escalates quality of care issues through established channels
+ Utilizes negotiation skills to secure appropriate options and services necessary to meet the member's benefits and/or healthcare needs
+ Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behaviour changes to achieve optimum level of health
+ Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices
+ Helps member actively and knowledgeably participate with their provider in healthcare decision-making
+ Monitoring, Evaluation and Documentation of Care:
+ Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
**Experience**
+ One year Case management experience A MUST / Case Management Certificate (Preferred)
+ Long term care experience (Preferred) Microsoft Office including Excel competent
+ Bilingual - Spanish / English (required)
**Preferred Qualifications:**
+ Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
+ Effective communication skills, both verbal and written
**Education**
+ Bachelor's degree required - No nurses. Social work degree or related field preferred.
