**Job Summary and Qualifications**
Role Summary: Interprets and evaluates appropriateness of physicians' orders and is responsible for accuracy of final product. Communicates drug information to all customers and provides pharmaceutical care.
**What qualifications you will need:**
+ Required Education: Bachelor's degree in Pharmacy
+ Preferred Education: A residency and/or Pharm.D. degree is preferred

- Preparation and compounding of sterile products?

- Complete order entry and refills under the supervision of the pharmacist?
- Inventory management and storage, complete appropriate documentation, such as labels, log sheets and reports?
- Participate in on call rotation as indicated by staffing and business needs?
- Have excellent written and verbal customer service skills and advanced computer skills in order to interact with key partners and patients
- Additional licensure may be required in multi-state service areas?

The typical pay range for this role is:
$17.00 - $31.30
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Complies with all sterile compounding regulations, standards, and policies. (i.e. USP 797 and USP 800). Calculates dosage volumes, converting between metric and apothecary equivalents.
+ Helps to ensure accurate documentation and billing to ensure compliance. Works with pharmacist and prescriber to resolve rejections and/or questions to facilitate the timely disposition of orders.
+ Maintains a safe, clean, and sterile work area in accordance with established policies and procedures. Performs and documents daily, weekly, and monthly cleaning practices and environmental testing. Disposes of all materials per policies and procedures that are used when mixing hazardous or non-hazardous IVs.
+ Follows all procedures for documenting and dispensing controlled substances. Handles controlled substances in a manner that ensures accountability and security.
+ Assures medications are available for patients by assisting in inventory control including ordering, checking in orders, stocking shelves, updating the inventory system, rotating stock, and processing recalls.
+ Conducts periodic inspections where sterile compounding components are stored in the pharmacy for outdated and unusable components and quarantining them in a separate area from usable stock.
+ Works in a constant state of alertness and safe manner.

**Responsibilities**
Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provides guidance across pharmacies and to clinical leadership.
+ Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
+ Recommend formulary alternatives when appropriate.
+ Compound and dispense medications for accurate delivery to patients. Provide drug information and education to medical staff and patients.
+ Communicate with physicians and other clinicians as necessary to facilitate care.
+ Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
+ Assist with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs. Provide patient counseling when required.
**Requirements**
Licensure / Certification / Registration:
+ Pharmacist credentialed from the Texas State Board of Pharmacy required.
Education:
+ Bachelor's or advanced degree in Pharmacy required.
**Additional Preferences**
+ 3-5 years IV room experience strongly preferred
+ Board certification in sterile compounding (BCSCP) strongly preferred

Job Description
The Pharmacy Technician demonstrates consistent application of knowledge and skills in assisting the pharmacist in execution of appropriate, safe, efficacious, efficient, and cost-effective pharmaceutical care. The position participates in many procedural aspects of pharmacy practice under the supervision of a licensed pharmacist or technician supervisor and is an integral part of the pharmacy team. This position supports Pharmacy Services in all locations (i.e., acute, community, ambulatory, specialty).
**The Shifts will be 0700 - 1530 Monday through Friday. Must work a minimum of 2 holidays per calendar year.**
**Essential Functions**
+ Assists with all phases of pharmacy services while under the supervision of a pharmacist as required by state of practice and job duties.
+ Answers phones and provides excellent customer service as appropriate to practice site.
+ Assists the pharmacist in collecting clinical and other patient information and maintaining appropriate records as required by regulation and policy.
+ Utilizes pharmacy technology effectively to prepare medication orders for dispensing, performs calculations as necessary, and compounds medications for accurate compounding as appropriate to practice site, in accordance with state law, policies, procedures, and protocols.
+ Attains appropriate supervision and accuracy checks by the pharmacist(s) and clearly understands when a pharmacist must verify tasks and activities.
+ Accurately fills and maintains medication supplies including verifying and monitoring controlled substance inventories to ensure security, accuracy, cost effectiveness, and 340B/Charging management as required to practice site.
+ Packages and labels medications accurately and appropriately to minimize waste as appropriate to practice site and job duties.
+ Ensures pharmacy cleanliness and orderliness as appropriate to practice site.
+ Communicates with caregivers regarding responsibilities, identified concerns, and participates in quality and patient safety initiatives, identifying potential safety hazards that may create problems for patients and/or staff and takes appropriate actions to escalate items to the pharmacist for review as appropriate based on urgency, regulations, and scope.
+ Participates in departmental meetings, contributing ideas or feedback and identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction and seeks a team approach to developing and achieving best practices and processes.
+ Promotes mission, vision, and values of Intermountain Health, and abides by service behavior standards.
+ Delivers detail-oriented, consistent, flexible care and completes advanced tasks consistent with scope and performs other duties as assigned by management.

- Follows all procedures for documenting and dispensing controlled substances. Handles controlled substances in a manner that ensures accountability and security.
- Assures medications are available for patients by assisting in inventory control including ordering, checking in orders, stocking shelves, updating the inventory system, rotating stock, and processing recalls.
- Conducts periodic inspections where sterile compounding components are stored in the pharmacy for outdated and unusable components and quarantining them in a separate area from usable stock.
