**Occupational Health Advanced Care Provider (NP/PA)**
**Site of Service:**
5th Medical Group, Minot AFB, ND - 194 Missile Avenue, Minot AFB, ND 58705
**Hours:**
Monday through Friday, (8-hour shifts). 40 hours per week with a 60-minute uncompensated lunch break. No on-call required. Contract workers will not be required to work on federally recognized holidays. Installation training days and family days observed at the facility are non-duty days on which contractor personnel may not bill for services.
**Position Overview:**
The Occupational Health Advanced care provider supports the 5th Medical Group at Minot AFB under a Defense Health Agency (DHA) personal services contract. This role provides comprehensive primary and occupational health care to active-duty Air Force personnel, including periodic health assessments, deployment-related health evaluations, and direct patient care across a broad range of acute and chronic conditions. The NP may serve as Primary Care Manager (PCM) for an assigned patient panel. The position offers a structured Monday-Friday schedule with no on-call requirements and malpractice coverage provided.
**Position Qualifications / Requirements:**
- **Certifications:** Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) required.
- **DEA Registration:** DEA registration number required.
- **Experience:** As required to meet clinical competency requirements specified in Service-specific credentialing instructions.
- **Citizenship:** U.S. Citizenship required.
- **Language:** Professional proficiency in English required, both written and verbal.
**Physician Assistant:**
- **Education:** Bachelor's degree. Graduate from an accredited college or university certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA). Equivalency determinations may not be substituted for this education requirement.
- **Licensure:** Current, full, active, and unrestricted license as a Physician Assistant (NCCPA) in any U.S. state, DC, Puerto Rico, or U.S. Territory.
- **Board Certification:** Certified by the National Commission on Certification of Physician Assistants (NCCPA).
**Nurse Practitioner:**
- **Education:** Master's degree in nursing. Graduate from an academic program within the nursing specialty field of study accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). Equivalency determinations may not be substituted for this education requirement.
- **Licensure:** Current, full, active, and unrestricted license as a Registered Nurse in any U.S. state, DC, Puerto Rico, or U.S. Territory.
- **Board Certification:** Certified in the applicable specialty as a Advanced care provider by the American Nurses Credentialing Center (ANCC), American Academy of Advanced care providers (AANP), or the recognized national nursing certification organization for the specialty area of practice.
**Duties:**
- The Advanced care provider shall provide occupational health care and primary care to
beneficiaries assigned to 5th Medical Group. These tasks include performing primary
care and occupational health clinician duties and the associated administrative tasks.
- Care shall include but not be limited to continuing, comprehensive health maintenance
and provision of medical care, including preventive medicine, behavioral health, occupational health, and community health.
- The Advanced care provider may become the primary care manager (PCM) for a panel of
patients. As a PCM, the flight medicine physician will be the primary person
responsible for the management of the health and wellness of his/her assigned patients.
- Duties include but are not limited to:
1. Examination of patients, formulation of differential diagnostic plans, ordering of appropriate diagnostic testing.
2. Interpretation of examination findings and test results, and implementation of treatment plans.
3. Determination of the need for consultation and assisting in medical care and treatment provided at the direction of other specialists.
4. Approving/disapproving subspecialty referrals.
5. Directing case management activities.
6. Answering patient telephone consults with the assistance of clinic staff.
7. Providing primary and secondary preventive maintenance care.
- Diagnose and treat a wide range of acute and chronic diseases and injuries including but not limited to conditions of the:
1. Skin Pulmonary System
2. Head Gastrointestinal System
3. Eyes Musculoskeletal System
4. Ears Genitourinary System
5. Nose Reproductive System
6. Throat Lymphatic System
7. Neck Central Nervous System
8. Cardiovascular System Endocrine System
9. Psychiatric and Behavioral Health
- Perform medical procedures and maintain certification where appropriate to
include, but not limited to, the following:
1. Basic Life Support
2. Simple minor surgical procedures: punch biopsies, excision of skin lesions
3. Simple abscess incision and drainage
4. Nail trephination
5. Sling or swath injuries
6. Suture simple laceration
7. Suture removal
8. Cryotherapy
9. Cultures (throat, wound)
10. Remove ocular, nasal and ear foreign bodies
11. Clearing of ears by flush technique
12. Reduce simple dislocations and fractures, when appropriate
13. Bandaging of sprains, minor burns, and minor lacerations
14. Administer intravenous, intramuscular, and subcutaneous medications as
appropriate
15. Stabilize and evaluate cervical spine injuries as appropriate
16. Splint and stabilize traumatic injuries to extremities
- Review, interpret, and act upon medical surveillance data relating to the
Occupational Health program.
- Prepare records and reports as required in support of services rendered in
accordance with established procedures.
- Attend and participate in patient care reports, review meetings, patient care
conferences, team conferences, professional staff conferences and other appropriate
professional activities only to the extent that such attendance and participation is
relative to assigned cases and/or performance of services.
- Conduct special operational evaluations and determinations, including (but not
limited to):
1. Apply medical, fitness and profiling standards IAW AFIs 48-123, and
10-203 (including profiling and duty restrictions as Profile Officer).
2. Complete deployment health assessments IAW DHA policy.
3. The Flight and Operational Medicine Clinic Flight Commander will assign specific
duties/patient care assignments, and other duties deemed necessary.
- In addition to standard mandatory qualifications, the healthcare worker must:
(1) Have experience with completing tasks using Microsoft Office applications with
minimal guidance.
(2) Become proficient in the use of the following computer systems: MHS Genesis, and
Aeromedical Services Information Management System (ASIMS).
(3) Complete SHPE Program orientation with a flight representative. This is a one-time
training requirement.
(4) Assist BOMC with medical record reviews for Deployment- Related Health
Assessments (DRHA), Separation History and Physical Exams (SHPE) and all other
Medical Clearances for Deployment, PCS, Retraining, Palace Chase/Palace Front, and
retention.
(5) Act as Competent Medical Authority (CMA). Training will be provided by the
government. CMA will act as medical consultant to the Personnel Reliability Program
(PRP) commanders.

**Special Requirements / Skills:**

Must complete SHPE Program orientation with a flight representative (one-time requirement). A National Provider Identifier (NPI) is required. **POSITION OVERVIEW:**
The Physician Assistant - Family Medicine will provide a full range of primary and specialty medical care services to active-duty service members, dependents, and other eligible beneficiaries at the 366th Medical Group, Mountain Home Air Force Base, under a Defense Health Agency (DHA) personal services contract. The Physician Assistant works collaboratively with physicians, nursing staff, and ancillary providers to deliver screening, diagnosis, treatment, and preventive care for routine, acute, and chronic conditions across all organ systems. This is a designated mission-essential position supporting the 366th MDG's readiness mission.
**POSITION QUALIFICATION/REQUIREMENTS:**
**Education:** Bachelor's Degree and graduate of an accredited college or university certified by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).
**Board Certification:** **Certification:** Certified by the National Commission on Certification of Physician Assistants (NCCPA).
**Licensure:** Current, full, active, and unrestricted license as a Physician Assistant from any U.S. State, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or U.S. Virgin Islands.

**DUTIES:**
· Provide a full range of Physician Assistant services in primary and specialty medical care within the limits of their training and privileges granted by the MTF (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated).
· Technically direct and provide care for a variety of diseases and types of injuries (i.e., acute, chronic and minor trauma), and the skills to recognize emergency conditions in order to diagnose, prescribe for and treat diseases, disorders and injuries, and refer the more complex cases and serious conditions to qualified medical personnel and perform designated treatment.
· Technically direct and teach other staff, provide educational lectures and participate in in service training to staff members.
· Promote preventive care and health maintenance including annual physicals, positive health behaviors and self-care through both formal and individual education and counseling.
· Prescribe in accordance with approved treatment protocols and the Pharmacy and Therapeutics Committee approved medications and immunizations.
· Identify a medical problem and determine appropriate action to meet the problem, including referral to a physician.
· Perform specific diagnostic and therapeutic practices and procedures in accordance with scope of practice.
· Establish collaborative rapport with physician, nursing and others in order to promote and facilitate primary care services.
· Teach patients and family member's healthy regimen and responsibility for achieving wellness.
· Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement.

**CORE DUTIES:**

· Determine the correct method of decontamination to include the following: removal of blood, tissue fragments, body fluids and other gross contaminants. Decontamination methods shall include wiping, soaking, rinsing, scrubbing and using ultrasonic and washer decontamination/sterilizer equipment.
· Examine all items after decontamination/pre-sterilization procedures to ensure that gross contaminants have been removed and that instruments function properly.
· Ensure that all medical devices and surgical instrumentation are properly labeled and identified.
· Assemble and prepare the full range of specialty trays and instrument sets. Select correct wrapping or packaging material for medical devices and surgical instrument sets, select sterilization process in accordance with standard MTF procedures to include: Ethylene Oxide (ETO) or Steam Sterilization.Correctly label, seal, wrap or place in rigid containers, items with appropriate chemical indicator tape, and aerate gas sterilized items for the correct period of time.
· Receive sterilized trays, sets, and equipment from the sterilizer; check for integrity of sterilization, (i.e., color changes on indicator tapes, external and internal indicators, and dampness of packages); stock and properly store sterile items in sterile storage room in accordance with MTF procedures.
· Assist with decontamination, cleaning, assembling, packaging, sterilization, storage and issuing of medical/surgical instrument sets, instruments and supplies.
· Provide wards, clinics and other users with sterile supplies and other SPD issued items.
· Process instrument sets delivered from wards and clinics and reissue them as required. Maintain established supply levels and rotation of supplies.
· Clean the SPD work areas per SPD standard operating procedures. Issue SPD supplies to sections and outpatients and properly complete hand receipts.
· Check supplies processed in SPD for expiration and proper packaging. Assist with the issuing, exchange and restocking of Crash Carts.
· Deliver broken medical items to Medical Maintenance. Receive supplies from logistics (100% inventory of supplies).
· Check medical supplies for serviceability. Assist with function check of sterilization equipment, and fold linen. Issue outpatient medical supplies to patients, and properly complete paperwork in the outpatient room.
· Assemble surgical case carts as directed by the daily operating room schedule or by request from the operating room service.
· Operate sterilizers, washer/sterilizers, sonic baths, sterrad sterilizers, cart washers, and other equipment items crucial to the processing of surgical sets and supplies.
· Maintain required SPD specific documentation to include sterilizer logs, equipment issue logs, case cart and crash cart documentation.
· Assemble case carts with surgical instrumentation and supplies for operating room and clinic use.
**HOURS:**
· Central Sterile Supply Technician services on a full-time time basis, 24/7, for the Department of Operative and Anesthesia Services.
· Scheduled work hours are up to 24 hours per day, 7 days per week, 365 days per year, to include weekends and federal holidays.
· Total work hours shall not exceed 40 hours per week, unless mutually agreed upon by the contract employee and Chief, Anesthesia Nursing or designee, in writing.
· Shifts may range from 8 hours to 12 hours at the contractor's discretion with approval from the Chief, Sterile Processing.
**BASE DOWN DAYS:**
· During a planned closure of the facility due to training, closures before or after holidays family down days, Employee will only be compensated for the actual hours worked. If clinic is closed and employee is not able to work, employee will be required to use leave or take LWOP (without penalty).
