Required Qualifications:

• • • Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S
    • Board certification or eligibility in Cardiology
    • Active, unrestricted medical license in Arizona
    • Valid DEA license
