Position
Overview:
Under the supervision of the Refugee Program Nurse
Coordinator, the Licensed Vocational Nurse (LVN) Navigator will
provide safe, compassionate nursing care as a member of the
profession, provider of patient-centered care, patient safety advocate
and member of the health care team. The Licensed Vocational Nurse with
a focus on patient safety, is required to function within the
parameters of the legal scope of practice of a LVN, and in accordance
with the federal, state and local laws; rules and regulations; and
policies, procedures and guidelines of HCPH.
Duties &
Responsibilities:
- Serves as a liaison for the newly arrived refugees between HCPH Refugee Health Screening program and the Local Resettlement Agencies and/or service providers, social services, and other healthcare providers
- Provide and explain laboratory results to patients and their families, and supplies individualized health education and counseling in a patient-centered manner. May be called upon to present group health education sessions when indicated.
- Assist with control of patient flow through the facility during clinic visits, including discharging patients from the clinic following the medical providers’ care plan and provides appropriate patient education.
- Monitors, tracks, schedule and follows up on abnormal test results/findings/missed appointments by telephone, postal correspondence, or home visits
- Ensures transportation and translation services are provided for the patients at their appointment
- Assists with patient screening (i.e. drawing blood, testing urine, taking/recording vital signs, heights and weights, vision and hearing assessment)
- Administers/dispenses prescribed medication according to Pharmacy D regulations
- Give age-appropriate immunizations, stay up to date with all CDC vaccine schedule changes, and assists the nursing team with providing CDC recommended vaccinations at all times and serves as a vaccination coordinator alternate for the Vaccine for Children’s (VFC) Program with the Texas Department of Health Services (DSHS)
- Assist in performing data entry and electronic data processing by using the program EMR, Microsoft Excel, and any other related software.

- Associates Degree or Diploma in Nursing
- Two (2) years of experience as an LVN in a public health or community health setting, or related field
- Current licensure as an LVN in the State of Texas by the Texas Board of Vocational Nurse Examiners
- Basic Life Support (BLS) Certified
- Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook) and navigating EMRs
- Excellent communication skills (verbal & written)
- Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
- Knowledge of current CDC recommendations for immunization
- Ability to draw conclusions from data, such as lab tests, to record this information accurately in patient’s record, and to notify the RN Coordinator or medical providers when deviations occur.
