Roles & Responsibilities:
Serves
patients by preparing medications; giving pharmacological information
to multidisciplinary health care team; monitoring patient drug
therapies.
Essential Duties and Responsibilities
1.
Provides pharmaceutical care to assigned patients by correlating the
physician's orders with the patient's clinical status to determine the
optimal dose (individual, daily, and cumulative), route, scheduling,
and duration of each prescribed medication.
2. Provides drug
information and/or references to health professionals and departments
within the hospital center and the community
3. Resolves
questions regarding formulary and non-formulary medications, following
policies and procedures.
4. Performs clinical chart reviews on a
scheduled basis and assesses the appropriateness of medication orders
based on interview, observation, and review of the patient’s medical
records.
5. Develops a goal-oriented pharmaceutical care plan
based on assessment, in cooperation with the patient, the patient's
significant others, and other members of the health care team
6.
Performs Patient Care Unit inspections on a scheduled and timely
basis.
7. Contributes to and participates in the departmental
medication use evaluation plan, including but not limited to CMI
(Clinical Management Initiatives)
8. Prepares parenteral
medications using sterile aseptic technique.
9. Maintains
adequate medication supply in the Pharmacy area and informs supervisor
of impending shortages
10. Intervenes and documents interventions
to correct/ improve pharmacotherapy. Reports adverse drug events and
medication errors occurring in area serviced.
11. Takes
responsibility for the operation of the assigned Pharmacy area and
activities of all supportive personnel.
12. Maintains
pharmacological knowledge by reading professional publications,
attending workshops, conferences and C.E. programs to maintain
professional competency.
13. Prepares patients for discharge in a
manner consistent with the patient’s status
14. Investigates and
documents reports of Adverse Drug Reactions. Facilitates Adverse Drug
Reaction Subcommittee meetings
15. Perform other duties as
assigned
Requirements:
Graduate
from an accredited College of Pharmacy (ACPE)
NYS Licensed
Pharmacist
