Registered Nurse Case Manager
Location: Santa Barbara and Goleta, CA
Schedule: 8am-5pm Part Time- 3 days per week (M-F)
Compensation: $55-58/hour
Responsibilities
+ Collaborates with patients' primary care team as needed to coordinate appropriate care
+ Retrieves messages in a timely manner and responds to patient needs according to medical priority
+ Reacts in a positive manner to patient concerns; remains calm and displays empathy
+ Works cooperatively with other staff and departments to problem solve and improve workflows
+ Maintains patient confidentiality/HIPAA
+ Participates in required meetings
+ Provides culturally sensitive, patient-centered case to all SBNC patients
+ Assists in the development, maintenance, revision and evaluation of clinic workflows
+ Elicits history of present illness and pertinent past medical history and uses clinical judgment and experience to formulate a care plan
+ Coordinates follow-up care for patients with recent emergency department visits or hospitalizations
+ Sends medication refills per clinic protocol
+ Utilizes health coaching skills and encourages patient activation and self-management
+ Help patients manage medications accurately and safely. RN is authorized to remove and update medications with Provider input
+ Discuss test results per provider order
+ Participates in outpatient case management of medically or psychosocially complex patients; visits may take place within the home. Maintains accurate charting and registry of all patients served under case management.

Santa Barbara Cottage Hospital seeks a Registered Nurse (RN) for their Neonatal Intensive Care Unit (NICU).

Located in the city of Santa Barbara, California, Cottage Children's Medical Center is recognized today as a regional referral center for the medical needs of infants, toddlers, and children throughout the tri-county area.

The primary focus of the 22 bed, Level III, Neonatal Intensive Care Unit (served by a dedicated RN/RCP Transport team) is the provision of critical nursing care to the infant in crisis. The neonatal patient may suffer from a wide variety of medical conditions such as prematurity, respiratory distress, meconium aspiration syndrome, hypoglycemia, hyperbilirubinemia, sepsis and surgical conditions requiring pre and post-operative care. Care ranges from the acute phase to patient needs or conditions necessitating close observation and interventions.

The NICU utilizes a comprehensive multi-disciplinary team approach to meet the patient's needs throughout their hospital stay and we recognize the importance of parents and family as being an important members of the team.

Job Description:

The Inpatient Registered Nurse provides hospice care and is skilled at assessing pain and other complex symptoms at the end of life. The professional nurse works with the interdisciplinary hospice team to plan and direct services to meet the needs of individuals and families in the General Inpatient setting. The Inpatient RN fully supports the Core Values and is accountable for upholding the Standards of Behavior of VNA Health.

Job Duties: 

• Responsible for providing comprehensive direct nursing care to Serenity House patients.

• Provide nursing services, treatments, diagnostic and preventive procedures requiring the skills of a registered nurse according to in-patient facility protocols.

• Attend and participate in interdisciplinary group meetings.

• Assists in development, implementation and revision of the interdisciplinary team care plan for each patient and family. Establishes goals, interventions including the patient and family in the process.

• Assess patient and report to the attending physician and appropriate interdisciplinary team members, response to treatments and medications as well as changes in the patient’s physical or emotional condition.

• Coordinate care with other interdisciplinary team members and with community resources as needed.

• Teach, supervise and counsel the patient/family regarding pain and symptom management and psycho-social aspects of care for the terminally ill.

• Assess and maintain patient safety.

• Teach the patient and family/caregiver self-care techniques as appropriate. Provide medication, diet and other instructions as ordered by the physician and recognize and utilize opportunities for health counseling with patients and families/caregivers.

• Assist the patient and family/caregiver and other team members in providing continuity of cared.

• Supervise LVN’s, Home Health Aides and Volunteers.

• Ensure that arrangements for equipment and other necessary items and services are available.

• Attend and participate in clinical education, inservice and nursing staff meetings.

• Participate in Quality Management and Utilization Review Activities.

• Maintain necessary records and reports in a timely manner.

• Applies working knowledge of state, federal local and accreditation regulations for the delivery of hospice services.

• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition. Records pain, symptom management changes, and outcomes as appropriate.

Qualifications:

• Current and active California RN license.

• Bachelor’s degree, preferred.

• Minimum one year acute care experience required; background in home health and/or hospice preferred.

• Current CPR certification.

• IV and Central Line experience desired.

• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.

• Basic working knowledge and skill of computers (email) required.

• Must demonstrate ability to communicate effectively, have initiative, dependability and good judgment.

• Must be able to collaboratively work as part of an interdisciplinary group, as well as independently and problem solve in a crises situation.

Santa Barbara Cottage Hospital seeks a Registered Nurse (RN) for their Neonatal Intensive Care Unit (NICU).

Located in the city of Santa Barbara, California, Cottage Children's Medical Center is recognized today as a regional referral center for the medical needs of infants, toddlers, and children throughout the tri-county area.

The primary focus of the 22 bed, Level III, Neonatal Intensive Care Unit (served by a dedicated RN/RCP Transport team) is the provision of critical nursing care to the infant in crisis. The neonatal patient may suffer from a wide variety of medical conditions such as prematurity, respiratory distress, meconium aspiration syndrome, hypoglycemia, hyperbilirubinemia, sepsis and surgical conditions requiring pre and post-operative care. Care ranges from the acute phase to patient needs or conditions necessitating close observation and interventions.

The NICU utilizes a comprehensive multi-disciplinary team approach to meet the patient's needs throughout their hospital stay and we recognize the importance of parents and family as being an important members of the team.

QUALIFICATIONS:

All job qualifications listed indicate the minimum level necessary to perform this job proficiently.

Education:

• Preferred: Bachelors of Science in Nursing Degree.

Certifications, Licenses, Registrations:

• Minimum: Current California Registered Nurse (RN) license
• American Heart Association (AHA) Basic Life Support (BLS)
• AAP/AHA NRP.

Years of Related Work Experience:

• Minimum: 1 year experience in a Level II or III NICU.
