Client Care & Case Management:

• Conduct in-home visits to assess client satisfaction and care quality
• Continuously update care plans based on changing client needs
• Build strong, trusting relationships with clients and their families to ensure the highest level of care

Caregiver Support & Field Training:

• Introduce caregivers to clients and provide hands-on training to ensure success
• Observe, coach, and guide caregivers during field visits to promote high standards of care
• Support new hires by instilling best practices, creating an environment of learning, and mentoring them

Quality Assurance & Compliance:

• Conduct regular quality checks and document field observations
• Ensure compliance with company standards and policies to maintain care excellence
• Collect client and caregiver feedback to drive continuous improvement

• Experience in Senior Care or Healthcare Management: Ideally, experience in home care or a similar field (CNA, LPN, or RN preferred, but not required)
• Excellent Care Planning & Client Assessment Skills: You should be skilled at managing care plans and ensuring high-quality service
• Confident Communicator & Supportive Leader: You'll need to build relationships with caregivers, clients, and families, so being a strong communicator and a team player is essential
• Ability to Work Independently: As this is a field-based role, you must be self-motivated and able to manage your time efficiently
• Valid Driver's License & Reliable Transportation: Willingness to travel throughout the service area
• Hands-on Care: Experience in caring for seniors (professional or personal) (CNA, LPN, or RN a plus) At times you will be assisting our clients with their ADLs

DIRECT HOME CARE WORKERUnder the direction of the Center Manager and/or Team Lead/Home Care Coordinator the Direct Home Care Worker, provides supportive services to the participant; including but not limited to in the home or community setting, during transport and on appointments, or and in the day health center. The PCA's responsibilities for each participant are specified in the Direct Home Care Worker Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. SPECIFIC DUTIES AND FUNCTIONS: •The Direct Home Care Worker's plan of care is based on the initial and on-going assessments that are performed by a home care RNCM.•The Direct Home Care Worker assists in providing and/or maintaining optimal physical and emotional comforts to frail elders, in the participant's home, community setting, day health center, and dementia unit, and during transport and on appointments.•The Direct Home Care Worker assists participants and their families toward achieving maximum self-reliance and independence within their environment, day health center, and dementia unit. •The Direct Home Care Worker performs the care or services that are outlined in the individual participant's Care Plan; any Care Guidelines that are pertinent to the participant's diagnosis are specified in the Care Plan. •The Direct Home Care Worker performs a variety of tasks and services for the participant, including: o Personal care; o Assistance with activities; o Assistance with meal preparation / feeding: o Assistance with toileting / managing incontinence; o Simple treatments; o Shopping; o Medication reminders; o Laundry services. o Extended hours care o Homemaking services / chores o Transportation and off site appointment support •The Direct Home Care Worker is responsible for accurate timely documentation as required by the service area they are supporting; including but not limited to the care and/or services provided on every visit and or encounter, transport and appointment, on the Direct Home Care Worker; the Direct Home Care Worker keeps the RNCM and or clinic nurse informed of the participant's progress. •Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor / manager.•The Direct Home Care Worker responds to emergency situations in accordance with established policies and procedures. •Works only within established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES: •Minimum High School Diploma or G.E.D. required. •Must have minimum of (2) years of relevant work experience. •Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience. •The Direct Home Care Worker participates in annual, required in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training. •The Direct Home Care Worker must possess the ability to establish and maintain interpersonal and interdepartmental relationships, and respect the physical, spiritual, and well being of participants and co-workers.•The Direct Home Care Worker must be able to organize and maintain flexible scheduling to coordinate job responsibilities. •Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.•Ability to relate to those participants with dementia and/or developmental disabilities.•The Direct Home Care Worker must be able to perform duties in an atmosphere of frequent interruption. •Must meet a standardized set of competencies (approval by CMS) before working independently. •Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. WORKING CONDITIONS: •Driving is required within PACE SEMI catchment area.•Must have reliable transportation available on a daily basis.•Must have current valid driver's license•Working conditions are variable. With exposure to communicable diseases, due to participant environment. There is also potential for exposure to chemicals such as cleaning solutions and medications. University Home Care is seeking a skilled and knowledgeable Medicare Skilled Home Care Intake Specialist to join our team at our Livonia, Michigan location. This is a full-time, hourly position in the healthcare/medical field. As an Individual Contributor, you will be responsible for coordinating and managing the intake process for Medicare skilled home care services. You will work closely with patients, families, and healthcare providers to ensure timely and accurate enrollment in our program.

Compensation & Benefits:

The Medicare Skilled Home Care Intake Specialist at University Home Care will receive a competitive hourly rate of $17 to $22 per hour, paid semimonthly. Responsibilities:
MUST Have 1 YEAR Experience
- Facilitate the intake process for Medicare skilled home care services by working closely with patients, families, and healthcare providers
- Gather and review necessary documentation to determine eligibility and medical necessity for skilled home care services
- Communicate with insurance companies and other third-party payors to obtain authorization for services
- Coordinate with our clinical team to develop and implement customized care plans for each patient
- Educate patients and families about Medicare coverage and our services
- Ensure all necessary paperwork and documentation is completed accurately and in a timely manner
- Maintain accurate and organized electronic records of all intake activities
- Collaborate with other members of the team to streamline processes and improve overall efficiency
- Adhere to all Medicare and Medicaid regulations and guidelines
- Participate in ongoing training and development opportunities to stay up-to-date on changes in policies and procedures

Requirements:

- High school diploma or equivalent, some college coursework in healthcare or related field preferred
- At least 1 year of experience in healthcare intake or related role, specifically with Medicare and/or Medicaid
- Excellent communication skills, both verbal and written
- Strong organizational skills and attention to detail
- Ability to work independently and in a team environment
- Proficient in Microsoft Office and electronic medical record systems
