As a Home Health Physical Therapist, you will play a crucial role in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes. This is a PRN position, meaning you will be working with us on an as-needed basis. You will be hired as a W2 employee, not as an independent contractor.

Key Responsibilities:
- Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs.
- Develop and implement individualized treatment plans in collaboration with the healthcare team.

- Educate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living.
- Monitor and document patients' progress, adjusting treatment plans as necessary.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.

Qualifications:
- Bachelor's or Master's degree in Physical Therapy from an accredited institution.
