Pediatric Therapeutic Services (PTS) is seeking a Speech-Language Pathologist (SLP) to provide school-based services for a local school district in Jacksonville, FL for the 25/26 School Year.

Responsibilities:
- Implement specialized therapy treatment, techniques, and instruction in accordance with Individualized Education Programs (IEPs) of students being served.
- Participate in Individualized Education Program (IEP) and multi-disciplinary team meetings to assist in evaluating student needs/progress and to develop and/or revise individualized therapy interventions, objectives, and goals.
- Collaborate with teaching staff to plan and implement classroom-based activities and instructional techniques to ensure carry-over of learning.
- Provide consultative services to parents in the therapy program of their child.
- Compile, maintain, and file all required student documentation and reports incompliance with deadlines, confidentiality laws, federal and state laws, State Board of Education regulations, and school administrative policies and procedures.

**Job Summary and Qualifications**
The Speech Therapist provides high-quality, patient-centered care by developing and conducting individualized therapy programs for patients with communication, cognitive, and/or swallowing impairments. As a member of the health care team, the Speech Therapist performs comprehensive patient assessments and establishes appropriate treatment goals. The Speech Therapist serves as an educator to patients, families, students and colleagues.
**What you will do in this role:**
+ You will conduct a subjective assessment of assigned patient's speech language abilities during spontaneous conversation. You will administer objective tests to assess patient's communication assets and liabilities.
+ You will structure and implement individualized patient program to improve communication abilities.
+ You will evaluate patient regularly during treatment and adjust program as needed to maximize patient progress.
+ You will document progress in patient's chart. You will complete notes, charges, discharge notes and patient communication.
+ You will consult with Program Director and Rehab Physician/Medical Director regarding pre-screen assessments and physician consultations to determine appropriate placement of potential patients.

Discovery At Home is hiring a Speech Pathologist/Audiologist to provide speech pathology services on an intermittent basis to clients in their homes and in an out patient clinic setting, in accordance with physician's orders, at our Westchase locations.
Responsibilities:
- Evaluates the client's level of functioning and hearing recommends mechanisms to enhance the client's hearing ability.
- Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establishes a Speech Therapy treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
- Assists in development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
- Assures that the physician's orders are appropriate and discusses necessary changes.
- Communicates with other team members and, when appropriate, instructs them in speech pathology techniques that they may use while working with the client.
- Teaches, supervises, and counsels the family and client in the total Speech Therapy program and other related problems of the client at home.
- Evaluates the client, his/her significant other(s) and the home situation to determine what instruction will be required, what assistance will be available for the family in caring for the client, and what other agency and community services will be required.
- Makes arrangements for out-patient services for procedures which cannot be given in the home.
- Records all evaluation data, treatments, and client's response to therapy intervention on a timely basis.
- Supervises the Home Health Aide every two weeks when Speech Therapy is the only professional discipline involved in the client's care.
- Records and reports to the physician the client's reaction to treatment and /or any changes in the client's condition.
- Participates in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the patient's status, and obtaining additional orders if necessary.
- Communicates with the physician at least every 30 days or whenever changes occur regarding the client’s status and obtains additional orders, if necessary.
- Evaluates the effectiveness of speech pathology intervention and updates the client care plan as needed.
- Writes clinical/progress notes on the date of the visit and incorporates same in the client's record within 72 hours.
- Assumes responsibility for self development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading.
- Confirms, on a weekly basis, the scheduling of visits with the Director of Nursing in order to coordinate the patient’s visit schedule.
- Participates in agency activities, case conferences and meetings, when appropriate.
- Participates with staff, clients, and physicians in discharge planning activities and completes a Speech Therapy Discharge Summary
- Understands and adheres to established policies and procedures of agency.
- Participates in Quality Assessment and Performance Improvement activities as assigned.
- Attends all patient care conferences as scheduled.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.
Assists in Agency Admission Process:
- Obtains a medical history, particularly as it relates to the present condition.
- Evaluates the client, and the home situation to determine what instruction and assistance will be available from the family in caring for the client, and what other Agency and community services will be required.
