The Occupational Therapist (OT) provides occupational therapy evaluations and treatment plans to participants. All participant care provided is in accordance with applicable InnovAge policies and state regulations.
  • Provides clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.

  • Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.
  • Performs other duties as assigned.
REQUIRED
  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA)
  • Bachelors in (OT), Masters in OT (MOT), Doctorate in OT (OTD)
  • National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
  • Registration with state in which practicing
  • Requires state issued driver's license, personal transportation, good driving record and auto insurance as required by law
  • A minimum of one year experience working with the frail or elderly is required.
PREFERRED
  • Two (2) years' experience, preferred

Job Description:

JOB DESCRIPTION
Summary/Objective: Ability to supervise adults who are charged with delivering medical/health services. Prefers leadership situations relating to meeting the social, emotional, psychological and academic needs of students. Prefers a work environment where flexibility, ability to deal with conflict, a high tolerance for stress, an ability to quickly internalize information and make informed decisions, and long hours are a part of the assignment.   • Promotes the student functional independence or participation within the educational environment by:

  • Improving ability to perform tasks for independent functioning if functions are impaired or lost.
  • Preventing through early intervention, initial or further impairment of loss of function.
  • Provides direct and indirect services that follow an established program based upon individual student educational needs.
  • Communicates and supervises other team members as well as family/caregivers.
  • Provide professional development to staff.
  • Maintains, completes and process records and documents required by Weld County School District 6.
