PRINCIPAL DUTIES AND RESPONSIBILITIES: As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.

  • Sets up, tests, and operates hemodialysis machines for patient treatments.
  • Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
  • Evaluates vascular access pre-treatment and performs vascular access cannulation.

  • Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
  • Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

EDUCATION AND LICENSES: High school diploma or G.E.D. required.

**PURPOSE** **AND** **SCOPE:**
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
Education/Communication:
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.

+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.

+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

Primary Responsibilities:

  • Maintains up-to-date knowledge of specific registration requirements for all areas, including but not limited to: Main Admitting, OP Registration, ED Registration, Maternity, and Rehabilitation units
  • Ensures complete, accurate and timely entry of demographic information into the ADT system at the time of registration
  • Properly identifies the patient to ensure medical record numbers are not duplicated
  • Responsible for reviewing assigned accounts to ensure accuracy and required documentation is obtained and complete
  • Meet CMS billing requirements for the completion of the MSP, issuance of the Important Message from Medicare, issuance of the Observation Notice, and other requirements, as applicable and documenting completion within the hospital’s information system for regulatory compliance and audit purposes
  • Collects and enters required data into the ADT system with emphasis on accuracy of demographic and financial information in order to ensure appropriate reimbursement
  • Carefully reviews all information entered in ADT on pre-registered accounts
  • Verifies all information with patient at time of registration; corrects any errors identified
  • Identifies all forms requiring patient/guarantor signature and obtains signatures
  • Ensures all required documents are scanned into the appropriate system(s)
  • Identifies all appropriate printed material hand-outs for the patient and provides them to the patient/guarantor (Patient Rights and Responsibilities, HIPAA Privacy Act notification, Advance Directive, etc.)
  • Follows “downtime” procedures by manually entering patient information; identifying patient’s MRN in the MPI database, assigning a financial number; and, accurately entering all information when the ADT system is live
  • Assesses self-pay patients for presumptive eligibility and when appropriate initiates the process
  • Monitors and addresses tasks associated with the Mede/Analytics PAI tool
  • Follows approved scripting, verifies insurance benefits on all patients registered daily by using electronic verification systems or by contacting payers directly to determine the level of insurance coverage
  • Thoroughly and accurately documents insurance verification information in the ADT system, identifying deductibles, copayments, coinsurance, and policy limitations
  • Obtains referral, authorization and pre-certification information; documents this information in the ADT system and submits notices of admission when necessary
  • Verifies medical necessity check has been completed for outpatient services.  If not completed and only when appropriate, uses technology tool to complete medical necessity check and/or notifies patient that an ABN will need to be signed
  • Identifies payer requirements for medical necessity
  • Verifies patient liabilities with payers, calculates patient’s payment, and requests payment at the time of registration
  • Identifies any outstanding balance due from previous visits, notifies patient and requests patient payment
  • Sets up payment plans for patients who cannot pay their entire current copayment and/or past balance in one payment
  • Thoroughly and accurately documents the conversation with the patient regarding financial liabilities and agreement to pay
  • When collecting patient payments, follows department policy and procedure regarding applying payment to the patient’s account and providing a receipt for payment
  • Clarifies division of financial responsibility if payment for services is split between a medical group and an insurance company.  Ensures this information is clearly documented in the ADT system
  • When necessary, escalates accounts to appropriate Patient Registration leadership staff, based on outcomes of the verification process and patient’s ability to pay

As an Associate Patient Care Coordinator with Phoenix Mental Health & Wellness and Northlight Counseling Associates , you will be responsible for providing support to patients and the clinical staff. The Associate Patient Care Coordinator will greet and support the check in and check out for patients, register new patients, prepare paperwork and review for accuracy, verify completion of consents and insurance eligibility, process payments for in-person appointments, handle existing patient communications, manage internal referrals and ensure work locations are presentable and welcoming. 

In 2025, Phoenix Mental Health and Wellness will be partnering with Northlight Counseling Associates to become Elevate Mental Health. Together, Elevate Mental Health will proudly provide comprehensive medication management and counseling services, both virtually and in-office, across four locations in Arizona. Our psychiatric care services focus on evaluating, diagnosing, and treating a wide range of mental health conditions. 

Location: 3839 N. 3rd St. (Suite 104), Phoenix, AZ

Schedule: Monday-Friday, 8am-5pm PST

Primary Responsibilities:

  • Review each patient’s chart prior to appointment and ensure consents and insurance verification is complete
  • Complete patients check in and check out; obtain registration forms, collect and upload insurance cards and identification, verify completion of consents and confirm insurance eligibility
  • Assist with onboarding new patients, help patient’s access patient portal, prepare and confirm completion of paperwork, ensure consents and insurance verification are complete
  • Process payments or co-pays, deductibles, co-insurance, self-pay, and missed appointment fees
  • Assist in update and/or scheduling of existing patients, including assisting with internal referrals initiated by the provider
  • Answer phone calls and return voicemails; record all requests in the EHR system for Physician follow-up, including refill requests
  • Support prescribers in filing prior authorizations for insurance and submitting and monitoring lab tests and results
  • Order and restock supplies
  • Monitor clinician/admin/OBC inbox and respond accordingly and take necessary steps
  • Ensure all locations are welcoming and adhere to brand guidelines

Patient Care Duties:

  • Obtain and record: vital signs, weight, medication profile, allergies, chief complaint, signature/initials where appropriate before appointment with Psychiatrist

Required Qualifications:

  • High School Diploma/GED (or higher)
  • 1+ years of customer service experience in a hospital, office setting, customer service setting, or phone support role
  • Must be 18 years of age or older

Preferred Qualifications:

  • Experience with Microsoft Office products
  • Experience in a Hospital Patient Registration Department, Physician office or any medical setting
  • Experience in insurance reimbursement and financial verification
  • Experience in requesting and processing financial payments
  • Working knowledge of medical terminology
  • Understanding of insurance policies and procedures
  • Ability to perform basic mathematics for financial payments

Soft Skills:

  • Strong interpersonal, communication and customer service skills

Physical Demands:

  • Standing for long periods of time (10 to 12 hours) while using a workstation on wheels and phone/headset

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $6.00 to 27.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
