The Transition of Care Coach is responsible for care coordination of our members who are experiencing a significant change in health status which has resulted in the necessity of an emergency department visit, inpatient, skilled nursing, or rehabilitative stay. Under the direction of a Registered/Licensed RN, the TOC Coach ensures the member experiences a seamless transition to their next care setting and facilitates post-discharge goal attainment by:
- Complete post-discharge questionnaire, which may be market specific.
- Ensures the member has filled/received their medication(s) and has an understanding on how to take their ordered medications.
- Benefit education
- Monitor members in low CM level for alerts or changes in condition to be transitioned back to RN.
- Complete post discharge call and required assessments (RAP), medication reconciliation (if within scope of practice), fall assessment if fall risk identified.
- Complete inpatient confinement calls and monitoring for discharge
- Management of warm transfers form concierge and engagement hub
- Provides clinical assistance to determine appropriate services and supports due to member's health needs (including but not limited to: Coordination with PCP and Specialty providers, Condition Management information and education, Medication management, Community Resources and supports)
- Evaluation of health and social indicators
- Identifies and engages barriers to achieving optimal member health.
- Uses discretion to apply strategies to reduce member risk.
- Facilitates overall care coordination with the care team to ensure member achieves optimal wellness within the confines of the member's condition(s) and abilities to self-manage.
- Coordinates post-discharge meal delivery, assists with securing DME, and helps to ensure timely physician follow-up.
- Understands Payer/Plan benefits, policies, procedures, and can articulate them effectively to providers, members, and other key personnel.
Our TOC Coaches are frontline advocates for members who cannot advocate for themselves. The TOC team will review prior claims to address potential impact on current case management and eligibility status. Focus assessments and/or questionnaires are designed to use a holistic approach to identify the need for a referral to clinical resources for assistance in functionality.
Additional responsibilities to include but not limited to the following:
- Responsible for completing outreach cadence calls and post-discharge questionnaires within required compliance driven timelines.
- Utilizes weekly and daily reporting to identify utilization for the purpose of reducing Emergency Department Utilization and 30-day hospital readmissions.
- Follows members identified as inpatient in hospitals (whether planned or unplanned admission) and then throughout the subsequent care continuum until member can return to prior level of functioning in the community.
- Facilitates Interdisciplinary Care Team Meetings with Social Services, Care Management, PCP and other key players to discuss service needs and support safe transitions.
**Required Qualifications**
- Active and unrestricted Licensed Practical Nurse (LPN) licensure in state of residence
- 2 years of LPN nursing experience
- Residency in Massachusetts, Connecticut, Rhode Island, Vermont, New Hampshire or Maine is required

The Social Work and Spiritual Care Clinical Practice Specialist is responsible for the oversight of the assessment, planning, implementation, and evaluation of care provided by social work and chaplain staff in clinical operations in collaboration with their direct clinical leader. This role is directly responsible for the professional competency oversight and professional development of social work and chaplain staff, students, and hospice interns. This role serves as a member of the Clinical Operations Team to facilitate the best practices and address concerns/problems associated with the delivery of clinical services.
This is a full time 40 hours a week position from 8:00am-4:30pm
What will you do as a Social Work and Spiritual Care Clinical Specialist?
- Represents HopeHealth Hospice in public forums, professional forums and partnerships which include exposure across multiple platforms/venues.
- Represents psychosocial disciplines and end of life care, as a Subject Matter Expert.
- Develops the implementation of practice theory into practice application.
- Supervises and/or oversees provision of state regulated supervision, and/or coordinates other supervision resources, of master's prepared social work staff for advancing to independent or clinical licensure and/or to meet the state requirements for provision of required supervision hours monthly.
- Performs gap analysis for organizational needs in Social Work and Chaplain practice and plans mitigation strategies in collaboration with Clinical Operations leadership to align with strategic initiatives.
- Orients and onboards new Social Workers and develops orientation and onboarding plan for new Chaplains. Assigns preceptor for new Chaplains and monitors onboarding process.
- Carries a patient caseload and performs patient assessments and follow up visits. Caseload size will vary depending on needs of the organization.
- Participates in Social Worker on call rotation.
- Performs chart audits of discipline specific documentation and coaching staff on best practices of documentation based on audit outcomes. Coordinates with clinical management when remediation is needed.
- Performs competency assessments and contributes to annual performance evaluations, for all Social Workers, in collaboration with clinical management which may also include remediation or performance improvement plans for staff.
- Serves as a mentor to new and current employees in a capacity which may require direct patient care. Through mentorship, mitigates practice gaps; moves clinical employees from the position of novice or intermediate clinician to a skilled clinician in end-of-life care with an expectation that all clinical staff practice at the highest level of competency for their discipline and according to the standards of practice.
- Develops preceptors to ensure learning outcomes for new psychosocial staff or students is achieved.
- Supervises social work students to meet their on-site learning objectives in collaboration with their institutions for social work intern placement requirements when applicable. Coordinates with Clinical teams for students to engage in patient care experiences.
- Collaborates with appropriate departments (such as IT) for ongoing practice enhancements in the use of the Electronic Medical Record. Trains staff on the use of the EMR and documentation standards. Performs audits and quality checks in support of performance improvement related to EMR use.
- Functions as a resource to clinical managers and staff, supporting specified teaching needs in the area of safe and effective delivery of care and continues to develop protocols for best practices in areas of expertise.
- Provide Just-in-Time Training when remediation needs are identified, or performance improvement activities are required.
- Assists with performance competencies (discipline-specific), skills checklists, and other daily team operations as assigned.

- Master's in social work (MSW) from an accredited educational institution with LICSW certification plus two or more years' experience in clinical practice.
