Key Responsibilities:
- Utilize both virtual and in-person methods to coordinate care with patients' medical providers and collaborate with other mental health professionals as needed.

Required Qualifications:
- Valid license in Tennessee as either an LCSW, LMSW, LAPSW, LMHC, LPC-MHSP, LPC-MHSP-T, LMFT, or LMFT-T.

JOB DUTIES

Responsibilities:The Clinician is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. Clinician will be responsible for patient caseload and will provide group, individual, and family therapy. Clinician I will conduct psychosocial and bio-psychosocial assessments. Clinician I will collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinating discharge planning. The Clinician I has primary responsibility for maintaining communication with the family, referral source, and other service providers as needed.
Core Job Duties
- Provides excellent individual, family, and group therapy to assigned patients following program curriculum provided by Clinical Director.
- Completes and ensures appropriate sign-off of all required documentation ensuring compliance with all company policies and procedures, state regulations, Joint Commission standards, and Medical Needs Criteria. This includes the following documents:
Biopsychosocial Assessment and Integrated Summary (72 hours)
Comprehensive Problem List
Integrated Master Treatment Plan and Updates (24-72 hours based on payer)
ASAMs (24 hours, based on state standards)
Individual, family, and group therapy session notes (24 hours)
Discharge Summary (72 hours)
- Uses substance abuse and co-occurring therapeutic interventions and de-escalation techniques to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
- Assesses patient for substance use disorders, risk to self or others, and co-occurring mental health disorders.
- Uses therapeutic interventions to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
- Manages aftercare recommendations and support that will prepare the patient for maintaining successful recovery.
- Provides group therapy.
- Provides caseload management as per state requirements.
- Participates in crisis intervention with patients as needed.
- Participates with multidisciplinary teams such as Financial Case Managers to provide appropriate aftercare planning and securing resources.
- Completes documentation daily to meet metrics of 92% completion within compliance.

- At least two (2) years of clinical experience in mental health field (additional training may substitute for one year

Job Summary :
The Clinical Therapist (Personal Wellness Coach) serves to provide outpatient therapy, counseling and support to our clients experiencing difficulties with mental and behavioral health. This position often includes coordinating care with the client’s key players, navigators, peer support specialist, other community mental health professionals, psychiatrist, and primary care physicians to assist with obtaining treatment goals.
Minimum Qualifications
Education, Experience and Licensure:
- A Master’s Degree from an accredited college or university in psychology, social work, or counseling or other related human services field.
- Current and appropriate licensure/ certification issued by the State of Michigan, Licensed Professional Counselor (LPC), Licensed Psychologist (LP), or Licensed Master of Social Work (LMSW).
- Certified Alcohol and Drug Counselor (CADC) or Certified Advanced Alcohol and Drug Counselor (CAADC) or development plan eligible.
- 1-3 years of experience providing therapeutic treatment within child welfare population.
Qualifications & Skills:

- Demonstrate ongoing engagement efforts with participants difficult to engage.
- Available to assist in team crisis intervention with Bridge participants during and after established work hours.
- Completes clinical records and agency documentation in a timely manner in accordance with the policies and procedures of The New Foster Care.
- Serves as the advocate for the young adult as it relates to mental, physical, emotional health in the community.
- Complete administrative tasks related to assigned caseload and other work assignments as required by The New Foster Care.
- Provides assistance, support and leadership to other staff members through consulting, teaching and other means as appropriate and to coordinate activities when assigned to do so.
- Participants in staff training and professional development as identified and/or approved including developing a knowledgeable understanding of the Transition to Independence Process Model utilized in the Bridge Program.
Additional Work Requirements:
- Valid unrestricted driver's license, with proof of current registration and automobile insurance; reliable personal transportation suitable for safely transporting as required.
