Verland is currently seeking Direct Support Professionals ' to join our team. In this crucial role, you will be responsible for providing Exceptional Care to the individuals that call Verland their Home.
House Name: Center Grange
Location: Monaca, PA 15061
Shifts available:
Full Time- 32 hours
7am - 3pm Sunday, Monday, Friday and Saturday
The Direct Support Professional (DSP) is responsible for individualized care and ongoing support for people in our community with complex physical, medical and intellectual challenges. This is an hourly, non-exempt position.
Contributions:
- Enrich and support the lives of our residents while maintaining a safe and healthy environment
- Introduce and encourage skill development through active treatment activities
- Implement individualized programs; record all observations and daily progress notes.
- Participate in planning social, home, and recreational activities
- Assist with meals and perform cleaning and laundry duties in the house
- Transport clients to appointments and activities in Verland vehicles
- Bathe, dress, and assist clients with personal hygiene
- Housekeeping duties as needed
- Medication Administration
- Other duties as assigned

2 weeks paid training provided by Verland, to educate new hires on daily living skills, social engagement, personal hygiene skills, along with each individual's support plan/ personal development goals that are essential to improve the daily lives of the individuals that we serve. Completion of training is required at the start of employment.

Service Coordinators support adults (21+) who are Medicaid-eligible through Community Health Choices and have physical disabilities requiring long-term care. You will help participants living in nursing facilities or private homes by assessing their needs, coordinating services, monitoring well-being, and ensuring care plans are up to date.
Service Coordinator - Case Manager Key Responsibilities:
- Conduct comprehensive assessments and ongoing reassessments
- Link participants to needed and authorized services
- Develop and maintain service plans outlining strengths and needs
- Monitor services to ensure satisfaction, safety, and adherence to care plans
- Communicate and collaborate with participants and team members
- Ensure participants maintain Medicaid eligibility and assist with resolving issues
- Maintain accurate and timely documentation
- Provide crisis intervention and support when needed
- Bachelor's degree in Social Work, Psychology, or a related field with practicum experience, or
- Three (3) years of experience in a social service or healthcare-related setting, or
- Registered Nurse (RN)
Independent, ethical, adaptable, motivated, tech-savvy, resourceful, kind, and an active listener.
Requirements:
- Valid PA driver's license, reliable transportation, and auto insurance
- Act 33/34 clearances, FBI fingerprint clearance, drug screening, I-9, and Social Security card

Working Conditions:
This position includes travel throughout the assigned county, sometimes in adverse conditions. Remote communication is common. Must be able to lift at least 15 pounds.
