**Overview**
We're looking for an **RN** **Patient Care Manager** to join our team.You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. You will be responsible for maintaining a high performing hospice program including managing, overseeing, and directing patient care activities to ensure the delivery of quality patient care. This role is responsible for teaching, evaluating, and developing associates and ensuring adherence to company policies and procedures and compliance with local, state, and federal regulations and regulatory agencies.
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range - $130,000.00 - $145,000.00
+ Overseeing and managing rescheduled, declined, missed, and reassigned visit requests, including making decisions as to priority and staffing based on clinical judgment.
+ Responding to phone referrals/inquiries in a timely, professional, and compassionate manner, including triage of medical concerns within scope of practice.
+ Preparing for, serving on, and facilitating the Interdisciplinary Group (IDG), as assigned, including presiding over IDG, presentation of patients for review, coordination of minutes, review of patient charts to note and address any changes to patient condition and/or the plan of care, and soliciting input from various care team members
**About You**
+ At least three years' experience of direct patient care and two years' experience in hospice or home-based care setting
+ Graduate of accredited school of nursing; bachelor's degree in nursing preferred
+ Current Registered Nurse licensure in state of residence and requested employment
+ Demonstrates knowledge and compliance with accepted hospice principles and practice, including industry standards, regulations, and best practices (i.e., Medicare, Medicaid, JCAHO, ACHC), company policies/procedures, and understanding ofterminally ill patients and their families
+ Ability to apply knowledge of the unique needs of hospice patient and families from various socioeconomic backgrounds to provide appropriate advocacy and oversight

**PURPOSE AND SCOPE:**
Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).
**PRINCIPAL DUTIES AND RESPONSIBILITIES:**
Patient Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC) Adequacy Monitoring Program (AMP) Urea Kinetic Modeling (UKM) and regarding disease process/access.
Patient Treatment:
+ Welcome assigned patients and inquire as to their well-being since their last treatment.
+ Evaluate vascular access for patency.
+ Obtain necessary pre and post treatment vital signs and weight.
+ Perform vascular access evaluation pre-treatment.
+ Perform vascular access cannulation.
+ Perform administration of Heparin as delegated or as allowed by state law.

+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
+ Report any significant information and/or change in patient condition directly to the PCT III (if applicable) or nurse supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the PCT III (if applicable) or nurse supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the PCT III (if applicable) or nurse supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
Maintenance/Technical:
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
+ Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

**A Brief Overview**
As an entry position, the PCC I, is expected to learn and apply job skills, policies, and procedures to complete essential assignments and tasks. They should exercise judgment within defined guidelines to determine appropriate actions and seek assistance when needed.

**What you will do**
+ Patient Care: Perform patient care duties, including measuring vital signs, weight, height, rooming, drawing labs, assisting providers with medication reconciliation, and other special departmental tasks such as EKG, ear lavage, etc.
+ Clinical Administration: Handle check-in/out, virtual rooming for providers, vaccine administration, point-of-care testing, and specimen collection.
+ Clinic Maintenance: Manage weekly clinic preparation, clinic area stocking, medication, and supply ordering, and ensure timely completion of work assignments.
+ Communication and Coordination: Handle patient phone calls, prescription refills, and clinic support operations, manage CRM/In-Basket, and serve as a liaison between the provider, the patient, and the staff.
**Education Qualifications**
+ High School Diploma or GED equivalent AND one of the following:- (1) Certificate of completion or official transcript from a medical assisting program- (2) Completion of medical assistant training by a licensed physician or podiatrist- (3) Certification as medical assistant through a Medical Board of California - approved medical assistant certifying organization
**Experience Qualifications**
+ Entry Level - No previous experience required

+ All levels require possession of knowledge, skills and abilities (job competencies) sufficient to function effectively at the level to which assigned.
+ Knowledge of and ability to use and apply medical terminology in performing his/her duties.
+ Knowledge of coding and billing regulations.
+ Knowledge of computer systems and software used in functional area.
+ Ability to speak and write effectively at a level appropriate for the job.
+ Ability to solve problems and identify solutions.
+ Ability to demonstrate customer service skills in interactions with all patients, families and staff, including high volume and stressful situations.
+ Ability to work independently as well as an integral part of the patient care team.
+ Ability to follow instructions and standard operating procedures.
+ Demonstrated familiarity, knowledge and understanding of home health, practice care coordination, clinical care skills.
+ Ability to demonstrate business communication skills including speak and write effectively at a level appropriate for the job (including legible penmanship/handwriting, knowledge of grammar and syntax and ability to write clear and concise clinical reports)
+ Ability to learn and grasp and update clinical knowledge on a regular basis (through HealthStream classes, continuing education and other work related courses and training) to adapt to an ever changing healthcare environment.
+ Ability to learn and understand population health and disease population concepts and practices; inventory methods and practices; motivation learning; customer relationships service concepts and practices.

+ BLS - Basic Life Support required .

The PCC II responsible for facilitating smooth day-to-day clinic operations while developing the necessary skills to progress within our care delivery team.

+ Clinical Expertise Development: Undertake work in assigned areas to develop the necessary expertise within a given specialty area or clinic.

**Job Summary and Qualifications**
The Lead Patient Care Technician (PCT) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living. Under the supervision of the registered nurse (RN), the Lead PCT assists the nursing staff in providing patient care in accordance with the PCT's skills/training and the needs of the Department, and performs or assists with more advanced procedures based on one or more achieved specialty certifications. The Lead PCT complies with all hospital policies/procedures and observes the State Board of Nursing rules for licensed personnel. In addition, the Lead PCT assists with ongoing training and/or precepting of new and current employees as directed by the supervisor. The Lead PCT is responsible for ensuring new processes, equipment and other changes are communicated and/or taught to other PCTs.
**Your responsibilities will include:**
+ Assisting patients with ADLs (Activities of Daily Living), supporting mobility and comfort, checking vital signs, collecting specimens, preparing for procedures, and monitoring telemetry as needed
+ Documenting patient care accurately and communicating important updates to the nursing team to ensure coordinated, timely care
+ Helping maintain a safe, clean environment by following infection prevention practices, setting up and caring for equipment, and ensuring needed supplies are ready
+ Responding to patient needs through regular rounding and compassionate communication to promote comfort and trust
+ Training new team members, sharing updates on policies and quality improvement initiatives that enhance patient care
**What qualifications you will need:**
+ High School Diploma, GED, or equivalent required
+ Basic Cardiac Life Support required
+ (CNA) Certified Nursing Assistant required
+ 1 plus years experience at an HCA facility required
+ 2 plus years experience in an acute care setting preferred
