Healthy Communities is CHC's Public Health AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities also serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them. Service delivery will directly address the public health crisis in vulnerable communities and make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community based assistance. Additionally, AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services.
This is an AmeriCorps Member role - Oral Health Coordinator - that supports Improving Oral Health for Young Children in CT Initiative primarily in Hartford, Middletown, and New Britain areas.
Terms & Program Benefits:
+ One-year service term from September 2, 2025 thru August 28, 2026
+ Serve a minimum of 33 hours a week for a total of 1700 hours during service year
+ $23,700 stipend over the course of one year, paid bi-weekly
+ An education award of $7,395 at the end of successful service term completion

As a Oral Health Coordinator for the Community Health Center, Inc., you will:
+ Conduct monthly patient health education workshops; monitor the completion of pre-post surveys by workshop participants; and complete data entry for workshops and surveys.
+ Track and document all completed patient engagement activities in patient's electronic health record.
+ Develop community outreach and patient engagement strategies to support patient registration for oral health care services.
+ Provide ongoing tabling and patient recruitment activities at main sites, specifically Hartford, New Britain, Middletown, and New London.
+ Provide 1:1 patient education to existing CHC patients on the importance of oral health care and establishment of a dental home.
+ Administer and collect PRAPARE (Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences) survey from caregivers of patients ages 6 months to 5 years of age at selected clinic sites.
+ Research, design, and develop resource list/content for caregivers of patients to address barriers in accessing services.
+ Connect caregivers of patients with resources needed to establish a dental home, such as transportation services.
+ Review and analyze PRAPARE data to formulate presentations and inform continued oral health services.
Qualifications:
+ High school or equivalent required
+ Proficiency in Microsoft office and internet-related applications
+ Excellent time management and organizational skills
+ Excellent oral and written skills
+ Demonstrated ability to problem solve and remain calm during a crisis
+ Successful clearance of all required criminal history checks (NSCHC)
+ Able to travel between CHC sites and in state
Preferred:
+ Associate's degree in public health, social work related field
+ Bilingual Spanish/English
+ Knowledge of and/or experience in behavioral health
+ Experience working with children and/or oral health

Within the Division of Community Oral Health, there may be opportunities for candidates seeking a full-time faculty position on the Clinician Educator or Tenure track at the Assistant, Associate or Full Professor level. Faculty would teach clinically and didactically within the Department and Division as well as engage in scholarly activity.

Qualifications for the Division Chief position include DMD/DDS/MD and/or PsyD/PhD degree or equivalent; experience with teaching and mentoring students and faculty preferably in interprofessional settings, demonstrated experience in research and scholarship with a track record of funding, and experience in the management of department/program operations. Preference will be given to candidates with public health training at the masters or doctoral level, and experience in community-based centers. Candidates seeking clinical practice opportunities must be eligible for unrestricted Pennsylvania dental licensure.

Our Hygienists are responsible for delivering direct dental care and associated services to patients under the general supervision of a dentist and for providing general support to the office.
- Deliver quality and compassionate care to every patient
- Deliver direct patient care to patients, including scaling and prophylaxis, x-rays, application of sealants and fluoride, and related procedures, under the general supervision of a dentist
- Provide excellent customer service by looking for opportunities to offer Same Day Care
- Educate patients and parents in oral hygiene instruction
- Assess the dental condition and needs of patients and review patients' medical history
- Responsible for adhering to all government regulations and company standards
- Guide parents and patients through our entire treatment process
- Assist with dental and perio charting; when necessary
- Take x-rays and save into patient charts, when necessary
- Provide memorable patient experiences by showing compassion for our patients and parents
Requirements:
- Current licensure as a Dental Hygienist in good standing
- CPR (BLS) from the ARC or AHA, including hands-on skill assessment (required upon hire)
