General Summary: Assist in promoting a child's mobility, quality of life, independence, and overall development within the home and the community by working with the family to address their priorities.
Primary Duties and Responsibilities:
• Provide evidenced-based Early Intervention physical therapy services through the coaching approach.
• Provide information and education to families, caregivers, and team members in order to support a child's development during daily activities and routines, as well as within natural environments. Collaborate with a child's family or caregivers to increase competence and confidence, in order to support a child's gross motor and overall development.
• Attend Individualized Family Service Plan (IFSP) meetings and updates, participate in development of IFSP outcomes, and address IFSP outcomes with families during ongoing home visits.
• Complete documentation and required paperwork write reports, collect data, and maintain records.
• Complete ongoing assessments, including annual re-assessments, in order to determine continued eligibility for Early Intervention services.
• Assist families with a child's transition from Early Intervention services to the school district or another community program once the child turns 3 years old.
• Collaborate with team members to draw from their knowledge and skills, as well as plan for joint home visits, in order to help strategize with families, as needed.
• Link families to community agencies and resources, as needed.
