**Job Summary:**
The Community Health Center of Buffalo, Inc. (CHCB) is seeking a full-time Family Medicine Physician to join their diverse and well-established health center. **Responsibilities:**
- Evaluate physical and psycho-social health status via medical history and physical examination.
- Diagnose and treat medical conditions within the family medicine specialty.
- Interpret diagnostic tests, consultations, and recommend therapeutic measures.
- Provide education and preventive healthcare.
- Offer clinical direction to healthcare team members.
- Adhere to clinical policies, infection control principles, and confidentiality standards.
- Participate in quality improvement and regulatory compliance activities.
- Direct and evaluate medical students.
**Qualifications:**
- Medical license registration certificate for the state of New York.
- DEA license registration certificate for the state of New York.
- Computer literacy including proficiency in MS Word and Outlook.
**Preferred Qualifications:**
- American Board Certification.
- Basic and Advanced Cardiac Life Support Certification.
- CFP Authorization Number under NYS Worker's Compensation.

**Physical Demands:**
This role requires sitting, standing, walking, bending, and stooping. Evening work along with some Saturdays may be required.

Role Responsibilities
- Deliver comprehensive and continuous care for patients of all ages-catering to pediatric through geriatric populations.
- Oversee chronic disease management, preventive strategies, acute medical needs, and mental wellness concerns with a primary focus on patient engagement.
- Perform thorough physical examinations, analyze diagnostic results, and craft personalized care plans.
- Collaborate seamlessly with care coordinators, mental health professionals, and specialists to provide integrated health services.
- Engage in initiatives aimed at improving care quality and optimizing patient outcomes.
- Utilize electronic medical records (EMR) proficiently to maintain accurate patient documentation.
- Provide both in-person and virtual visits to meet patient preferences and requirements.
- MD or DO degree from an accredited medical institution.
- Completion of an accredited Family Medicine residency program.
- Board Certification in Family Medicine is required.
- Must hold (or be eligible for) an active medical license in New York.
- BLS and ACLS certifications are favored.
- Robust clinical and diagnostic abilities with a strong emphasis on preventive health.

Responsibilities:
- Provide full-spectrum family medicine care, including preventive care, chronic disease management, acute care, and women's health
- Deliver obstetric care including prenatal, intrapartum, and postpartum care
- Offer comprehensive, patient- and family-centered care.
- Ensure consistent, cost-effective care.
- Provide inpatient obstetric and newborn care as part of a call pool with other physicians.
- For surgical obstetric fellowship trained family physicians: perform scheduled Cesarean deliveries and bilateral tubal ligations
- Provide patient care in a holistic manner including attention to physical, mental, emotional, and spiritual health.
- Maintain electronic medical records efficiently and maximize resources.
- Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center.
- Create a learning plan that includes familiarity with new research and changes the standard of practice.
- Participate in quality improvement/population health activities with a data-driven approach. Participate in regular peer reviews and risk management activities as assigned.
- Attend required staff and clinician meetings except when clinical activities prevent such attendance.
- Arrange to ensure that mandatory and required training is completed
- Serve as a clinical preceptor or mentor to students and residents, as applicable
- Doctor of Medicine or Doctor of Osteopathy, licensed to practice in New York State prior to start date of employment
- Completion of three-year or four-year Family Medicine Residency
- Board-certified
- Passion for high quality, community rooted primary care among a culturally and linguistically diverse urban population
- Demonstrated ability to develop collaborative working relationships
- Comfortable working in a fast-paced, team environment
- Willingness to be flexible
- Proficient in utilizing electronic health records
- Reliable transportation within the city of Buffalo
Rate: $190,000-$220,000 salaried. Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
