Ascension Illinois is looking for a Board Certified/Board Eligible Family Medicine Physician for our Family Medicine Department.

**Responsibilities:**
+ Evaluates and treats patients with appropriate medical diagnostic and treatment skills.

+ Accepts patient referrals from other physicians. Acts as admitting, attending and/or consulting physician for patients who require hospitalization for primary medical diagnoses, depending on needs of patients, medical staff and hospital.

- Seeing patients with a broad array of patient needs; conducting a mix of acute, chronic, and well visits (not a panel-building role)

Major Responsibilities:
Clinical Care and Health Outcomes - Participates in the delivery of quality clinical care. Works closely with healthcare team and physician to assure triage, telephone assessments, home therapies and patient visits are consistent with established guidelines and followed by the healthcare team. Takes a lead role with the clinical team to meet/exceed CI goals. Assists physicians and associates in managing patient registries.
