**Job Summary and Qualifications**
The Case Management Manager works in conjunction with other members of the interdisciplinary care team using a proactive approach to facilitate effective and efficient patient care during the hospital stay.
+ You will facilitate effective and efficient patient care during the hospital stay. Face-to-face visit with patients and/or family members for comprehensive assessment for discharge needs taking into account the patient's needs, current and prospective functional capacities, resources and support systems.
+ You will follow-up visits and phone calls with patients and/or families as needed
+ You will complete accurate and timely utilization review of patient charts to insure appropriate utilization of hospital resources and facilities
+ You will provision clinical information to 3rd party payers in support of the patient's admission and/or continued hospital stay
+ You will have active communication with the physicians regarding issues such as length of stay, admission and/or treatment criteria, and how to render services in the most cost effective setting
+ You will complete a comprehensive assessment that takes into account the patient's needs, current and prospective functional capacities, resources and support systems.
+ You will have collaborative formulation of appropriate and realistic patient-centered discharge plans including the arrangement of any necessary follow-up services such as durable medical equipment, home health and/or placement
+ You will provide advocacy in support of the patient's right to self-determination
+ You will be acting as a liaison between patients, families, visitors, physicians, LPDS staff and/or other community service providers
+ You will provide community information and referrals
**What qualifications you will need:**
+ Registered Nurse
+ Bachelors Degree or Licensed Vocational Nurse with clinically focused work history.
+ 2+ years of supervisory or management experience within an acute care facility preferred

Job Description:
The Sr. Engineer will join the Facility Group to manage, plan, direct, and coordinate facility project activities to ensure projects are accomplished within the approved scope, budget, and schedule. The Sr. Engineer will oversee facility projects of varying sizes and complexity, ranging from greenfield/new construction to major renovations and minor upgrades of existing facilities. This position requires the ability to manage projects from the conceptual engineering phase through construction completion. Interaction with all Grifols' Departments, Vendors, and Contractors is critical for project success. The Sr. Engineer independently performs most assignments and provides direction for the project team to achieve facility project objectives and ensure resources are utilized efficiently. The position requires an understanding of project requirements to ensure projects are in compliance with cGMP design, applicable codes, and environmental laws, rules, and regulations.
Job Responsibilities:

Manage facility project design and construction by coordinating, reviewing, and administering the work of Architects, Engineers, Consultants, and Contractors.

Create cost management solutions for facility projects. Manage vendors, engineers, and consultants to ensure facility projects are completed on budget and per schedules.

Responsible for project management, project planning, budgeting, cost control, value engineering, and project cost analyses.

Review and evaluate bid proposals/estimates with procurement.

Manage facility projects and project planning efforts, including both new installations and upgrades to existing facilities.
