RN/LMSW- Case Manager Evenings
RN/LMSW Case Manager is responsible for the delivery of comprehensive nursing care to a set of assigned patients at a specific point in time. This involves the assessment of patient and care team needs and the development, implementation and evaluation of an appropriate Plan of Care, making changes in response to changing patient needs.
RN/LMSW Case Manager; guides the utilization review process using recognized standards of care to assure appropriate medical necessity, level of care, patient status and related notifications are met.
RN/LMSW Case Manager; guides patient progression across the care continuum to ensure safe and timely transitions to the appropriate level of care.
Roles and Responsibilities:
- Evaluates hospital admissions using evidence-based standards of care to assure care is medically necessary and delivered at the most appropriate level of care.
- Ensures patient demographics, insurance, primary care provider, and advanced directives are up to date in EMR.
- Asses the needs of the patient population in the ED and schedule appropriate outpatient follow up as necessary
- Maintains working knowledge of the organizational and department objectives and goals and takes actions to achieve targeted outcomes.
- Maintains current knowledge of federal, state and accreditation standards related to UR and Discharge Planning requirements.
- Maintains high visibility and is readily approachable.
- Is flexible in assuming other responsibilities not noted above.
Education - Graduation from accredited Nursing program or Masters of Social Work program.
Expected to complete Case Management Certification - or equivalent - within 18 months of hire
Experience - Minimum 2 to 3 years' experience in an acute care hospital setting or similar health care case experience.
Knowledge of community-based organizations and strong communication, organizational, critical thinking and problem solving skills required.
Computer literacy and knowledge of EMR systems.
Demonstrates resilience in coping with challenging situations as well as the emotional stability and organizational skills necessary to meet the demands of the busy healthcare environment.
Licensure - Current unrestricted NY State Licensure RN/LPN/LMSW
Preferred Skills and Experience:
- BSN in nursing preferred
- Previous Utilization Review experience

The Program Director for the CARS Withdrawal and Stabilization program in Ithaca, NY will oversee daily operations and lead the development and operations of our client centered, 24/7 Withdrawal and Stabilization programs. Working closely with executive leadership, this individual will take the initiative in identifying opportunities, solving complex challenges, and implementing innovative solutions to meet the needs of the community.

Roles and Responsibilities:
- Provide clinical and administrative oversight of the daily operations of the Withdrawal and Stabilization program; participates as an active, forward-thinking member of the CARS leadership team, providing leadership and direction in accordance with organizational goals and objectives. Serves as a resource to others as an extension of the leadership team.
- This leader integrates patient experience, operational effectiveness, and system outcomes by partnering with human resources on hiring, provider engagement, financial viability, business development, and clinical/operational workflows. The director will create a culture of growth through improved patient access, patient satisfaction, and supporting the continuum of care for a seamless patient experience.
- Direct and indirect supervision of the clinical, medical, milieu, and administrative teams. Ensure compliance with clinical supervision of counseling staff in accordance with OASAS requirements. Responsible for the fiscal integrity of the Withdrawal and Stabilization program including, but not limited to, maximizing revenue opportunities, ensuring programs operate efficiently and responsibly, monitoring and reporting of fiscal goals, being knowledgeable of the program budget and maintaining expenses within that budget and ensuring service deliverables are met.
- Assists in developing comprehensive training plans for new staff, covering clinical protocols BS documentation procedures. Assists in development, facilitation, and delivery of training sessions for staff to enhance their knowledge and skills in providing a safe and therapeutic, healing environment.
- Ensure programs maintain all required licensures as issued by OASAS, SAMSHA, DEA, DOH, and any other related licensures necessary for the efficient and effective operations of program; establish positive and collaborative rapport with representatives of the regulatory agencies.
- Collaborate with the Compliance. Quality, and Risk departments to ensure timely reporting and responses to incidents and/or compliance concerns, including monitoring corrective action plans.
- Maintain secure management of medications, medical records, and employee documentation.
- Uphold and protect patient confidentiality and rights, ensuring each patient receives proper medical and clinical care
Required Skills and Experience:
- Bachelor's degree in related field required.
- Must be a qualified health professional working within their scope of practice. This includes, but is not limited to, a registered nurse (RN), psychologist, certified nurse practitioner (NP), physician assistant (PA), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Certified Alcohol and Substance Abuse Counselor (CASAC).
- Must have at least 5 ears of full-time clinical work experience in the substance use disorder field, with at least 1 year of supervisory experience prior to appointment.
- Must have progressive leadership background, excellent communication skills, independent, self-starter, excellent problem- solving ability and experience working with people with substance use disorder. Familiarity with strength-based leadership models and practices a plus.
- General understanding of NYS Insurance standards, OASAS admission criteria and regulations including, but not limited to 816 and 820.
- General principles and practices of personnel management, supervision, and leadership development. General knowledge of the physical, mental, emotional, and social ramifications of substance abuse; general knowledge of substance abuse counseling techniques, group dynamics and techniques of conduction of psycho-educational and therapy groups; general understanding of the principles of adult learning; interviewing techniques; generally accepted crisis intervention techniques

- Master's degree in related healthcare field with knowledge of treatment of substance use and mental health disorders.
