- Conducts regular OASIS reviews for all patients.
- Meets with all direct care team members on a regular basis to provide guidance, coaching and communicate information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff.
- Communicates frequently with the Branch Director or Administrator on key performance metrics.

- Associate degree in a Nursing related field required

Home Health Nurse Responsibilities:
- Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
- Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
- Examine patients, do assessments, document progress and report changes to their physician
- Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
- Coach family members on the patient’s plan of care, medication and home safety
- Ensure goals are met and coordinate discharge from services

A few must-haves for Home Health Registered Nurses:
- Registered Nurse (RN) with active, unrestricted license in the State of Michigan – BSN not required

Position Summary:
Reporting to the Regional Director, the Clinic Manager's role is to lead the day-to-day operations of the clinic, including the treatment of patients. This may include onboarding new employees, training current employees, reviewing treatment plans and being an important part of the community you serve - ultimately enhancing our patient's health and quality of life.
