TheRehab Manager is responsible for overseeing daily clinical operations and ensuring that all home health services comply with Medicare regulations, accreditation standards, and internal policies. This leadership role supports the development of staff, promotes adherence to best practices, and ensures optimal patient outcomes through data-driven performance improvement initiatives. They will lead and support a team of clinicians in delivering Medicare-certified home health services. The Rehab Manager is also responsible for ensuring regulatory compliance, promoting clinical excellence, and maintaining operational efficiency.

Responsibilities for a Rehab Manager:

  • Provide direct supervision and support to field clinical staff
  • Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, and agency protocols
  • Monitor and manage quality assurance processes, including OASIS documentation, Plan of Care development, and clinical chart reviews
  • Facilitate staff orientation, education, and performance evaluations
  • Collaborate with the interdisciplinary team to ensure coordinated, efficient, and patient-centered care
  • Participate in the hiring, training, and retention of qualified clinical staff
  • Support survey readiness and lead corrective action plans following audits or compliance reviews
  • Track key performance indicators (KPIs) such as hospitalization rates, patient satisfaction, and documentation timeliness
  • Provide clinical support and case conference leadership as needed
  • Lead regular case conferences and foster a culture of collaboration and accountability within the clinical team

Qualifications for a Rehab Manager:

  • Current license in good standing, Physical Therapist (PT) or Occupational Therapist (OT).
  • 2 or more years of experience in a leadership/ supervisory role.
  • Excellent organization and communication skills.
  • Desire to lead, inspire, and motivate others.
  • Strong knowledge of home health policies and procedures
  • Demonstrated ability to manage clinical operations and lead teams through change

The Unit Manager, in accordance with the goals, policies, procedures and standards of his/her respective area(s) and the Hospital, is responsible for: supervising staff, managing daily operations of assigned Unit(s) 24/7. Ability to perform duties and documentation per departmental, DNV, PA Department of Health and other healthcare requirements. Working knowledge of PA. Laws impacting health care delivery, i.e. Advance Directives, Guardianship, Abuse and Neglect and of community resources.

Demonstrates knowledge and understanding of physiological and psychosocial implications to enhance the care of individual patients. Demonstrates ability to correlate medical necessity in achieving financial and quality of care outcomes.  Possesses strong knowledge of DRG's, LOS, variance analysis, and discharge planning related to the utilization of health care resources. Acute care and long term care knowledge base required.

Works independently.  Utilizes time management techniques in accomplishing assigned tasks. Recognizes patient and/or family level of understanding relative to medical information and communicates appropriately.  Possesses ability to negotiate and communicate with other departments, physician practice groups, 3rd party reviewers and outside facilities.  Exhibits professional written and verbal communications that are accurate and concise.  Possesses strong leadership skills and interpersonal skills.

LICENSE AND REGULATORY REQUIREMENTS:

Must have current RN licensure (BSN preferred)).

Underwrites risk (i.e., selection/rejection of new and renewal business). Analyzes quality and quantity of risks underwritten and prepares reports accordingly. Keeps informed of financial and market trends in the portfolio and analyzes those trends for the effect they will have on the portfolio.
Assists in special projects as assigned.

Qualifications:

7am-3pm Full time
3pm-11pm Part time
11pm-7am Per Diem

$1500 sign on bonus for 7am-3pm Full time

Key Responsibilities

Job responsibilities for Licensed Practical Nurses (LPNs) include and are not limited to:
  • Assisting CNAs in performing ADL and routine care.
  • Conducting resident/patient treatments.
  • Administering medication in accordance with physician orders and Plan of Care.
  • Submitting pharmacy orders.
  • Accurately documenting and recording all information.
  • Giving injections of medication as prescribed.
  • Observing resident health to communicate current condition to RNs, Supervisors, and Physicians.
  • Taking vital signs, including blood pressure, temperature and weight.
  • Basic care, including wound cleaning and bandaging.
  • Managing IVs, starting IV drips.
  • Monitoring fluid/food intake and output.
  • Observing and reporting resident accidents, incidents and the presence of skin breakdowns.
  • Assisting as directed in the admissions process.
