Job Overview:
The Nurse Practitioner will work one day per week seeing patients in-office at Roselle Park Medical Associates, with the potential for afternoon home visits as needed for established home bound patients within the area.

The position includes on-call responsibilities every fifth week, covering after-hours of 5:00 PM to 9:00 AM, Monday through Friday, and all day on weekends.

- Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching.)
- Manages medical emergencies as indicated, and according to standing orders.
- Provides monitoring and continuity of care between health care settings.
- Rounds regularly at one to two subacute care facilities.
- Performs transitional care visits for patients discharged from subacute/nursing home facilities.
- Conducts home visits as part of ongoing patient care.
- Preferred background or strong interest in palliative care.
- Completes POLST (Practitioner Orders for Life-Sustaining Treatment) forms.
- Collaborates closely with physicians within the practice to coordinate and align patient care.

Successful candidates should have the following:
- Amaster's degree in nursing from an accredited school or as grandfathered by New Jersey law required.
- Current licensure as a registered nurse in the state of New Jersey and certified by the State Board of Nursing.
- Certification within area of specialty required and granted privileges from Atlantic City Medical Center medical staff. NRNNP,C or RNCNS,C registry unencumbered.
- Minimum 2 years of experience in the field of specialty as an RN in the state of New Jersey.
- Proficiency in clinical applications preferred at time of hire. Incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from the date of hire.

About the Role
The Advanced Practice Nurse (APN) plays a key role in enhancing patient care and improving clinical systems within the Trauma Service. In collaboration with Trauma Physicians, the multidisciplinary Trauma Team, and other healthcare professionals, the Trauma APN delivers direct patient care to trauma patients. This role draws upon advanced clinical expertise to support patient management, provide staff orientation, consultation, and evaluation, and offer education to patients, families, and healthcare team members. The APN works closely with physicians and nursing leadership to assess and address both patient care and staff development needs. Additionally, the APN holds a clinical faculty appointment and serves as a preceptor for students. The role includes participation in staff development initiatives, patient and family education, performance improvement efforts, and trauma-related research and educational programs. All performance standards are monitored by the Director of the Division of Trauma and the Trauma Program Manager, unless otherwise specified.
Responsibilities
What You'll Do
- Enhances patient care and clinical practices within the Trauma Service

What You'll Bring
Required:
- Certified as an Advanced Practice Nurse (APN) in the State of New Jersey
- Master's Degree required
- Minimum of three (3) years of nursing experience required
- At least two (2) years of surgical intensive care or trauma RN experience preferred
- Experience caring for patients on research protocols preferred
- Understanding of clinical research principles is highly desirable
- Must be computer literate with strong organizational skills
- Excellent verbal, written, interpersonal, and group communication skills required
- Compassion and professionalism are essential
- Current certifications required:
- Basic Life Support (BLS)
