Job Description
The Nurse Practitioner will act as part of a care team that directly interfaces with a small panel of patients with CKD stages 3b, 4, 5, and ESRD in order to drive high quality value-based care. He/she will interact with and evaluate patients in the home, via teleconference, or via telephone as appropriate. In this role, the NP will create close working relationships with the patients served and with the care team members. Every day will entail contact with care team members and patients to coordinate optimal care for the patient panel.

Location: This role provides in-home care across the Pompano Beach, FL area. Nurse Practitioners operate as part of a care team and travel to patient residences. This is not an office-based or clinic-based position.

The Louis and Anne Green Memory and Wellness Center's mission is to meet the complex needs of persons with memory disorders, such as Alzheimer's disease and related dementias (ADRD), and their families through a comprehensive array of services, compassionate and innovative programs of care, research, and education. Summary of Responsibilities:
+ Initiates and leads the comprehensive memory evaluation clinical process conducted by an interdisciplinary team of geriatric specialists in consultation with the patient's personal physician/health care provider. The evaluation addresses the nature, extent, and cause of memory problems.
+ Performs an extensive, memory-focused history and physical assessment. This includes a review of all current medications, a screening of emotional factors that may affect memory, and a screening of cognitive functions, including memory. Meets with family members/caregivers for their concerns and questions.
+ Remains attentive and responsive to the patient's and the family member's/significant other's needs.
+ Works with the patient's existing health care providers to obtain necessary laboratory work and neuro-imaging studies.
+ Determines next steps in the process, such as appropriate (medically necessary) referrals for neuropsychological testing.
+ Meets with the patient and family at the feedback session to review the test results and recommendations. Forward evaluation reports to the patient's treating physician(s)/health care providers.
+ Leads and participates in the Clinic's interprofessional case conferences, held at least twice weekly, and prepares written reports to share with the patient/family and health care providers.
+ Works closely with the Clinical Director, the Center's Director, each member of the team/staff, the Day Center, the MD collaborator, and the Medical Director in order to meet the complex needs of the persons we serve, exemplifies our philosophy of the College of Nursing and guides students' practicum experiences.

+ Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate.
+ Introduces self to patient/family and explain nurse practitioner role.
+ Facilitates patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify post-SNF needs.
+ Enhances a collaborative relationship to maximize the patient's/family's ability to make informed decisions re: goals of care, palliative care and hospice.
+ Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.
+ Prescribes medication to patients based on State of practice.
+ Other duties as assigned and modified at manager's discretion.
**KNOWLEDGE, SKILLS AND ABILITIES:**
+ Demonstrated record of consistently achieving clinical performance metrics in current role
+ Strong Critical Thinking and problem-solving skills
+ Excellent communication and interpersonal skills
+ Time management skills with the ability to work well under pressure
+ Must be caring and empathetic and have great listening skills
+ Must be detail-oriented, and able to pay close attention to patient charts, medications, and follow-up on details of patient care
+ Basic computer skills and some knowledge of Microsoft Office Suite
+ This position may require 50-75% of local travel
**EDUCATION AND EXPERIENCE CRITERIA:**
+ ARNP or similar advanced degree in Nursing required
+ Current Nurse Practitioner Certification in the State of practice required
+ Board certification by AANP or ANCC is preferred but may be required for certain States
+ Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
+ A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
+ Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment

Advanced Practice Registered Nurse

Otolaryngology, Pediatric: Otorhinolaryngology

Full Time

The Advanced Practice Registered Nurse (APRN) delivers medical care to a wide variety of patients. The APRN also examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care.

Core Job Functions
  • Performs and documents complete physical examinations and comprehensive health histories.
  • Functions independently to perform age-appropriate history and physical for patients.
  • Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
  • Implements interventions to support the patient to regain or maintain physiologic stability.
  • Assists with the provision of care in accordance with facility, state, and federal regulations.
  • Monitors the effectiveness of interventions.
  • Facilitates the patient's transition within and between health care settings, e.g. admitting, transferring, and discharging patients.
  • Collaborates with multidisciplinary team members by making appropriate referrals.
  • Facilitates staff, patient and family decision making by providing educational tools.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Education: Master's degree in relevant field

Certification and Licensing: Valid State of Florida APRN License
