- Examine and treat chronic and acute episodic wound-related illnesses and accidents, minor injuries, and accidents.
- Interpretation of diagnostic tests and recommendation of appropriate follow-up.
- Refer to and update physicians, write prescriptions, and perform bedside procedures associated with wound care.
- Work with the medical director in the care and treatment of residents and staff.
- Debridement of wounds by sharp, enzymatic, autolytic, or surgical methods is able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
- Application and management of topical wound care and associated wound management products.
- Direction and recommendation of associated services to improve wound healing outcomes.
- Educate patients and Primary Care Physicians on current wound management practices.
- Proof of Covid vaccination.
- Must be currently licensed as a registered nurse in the state.
- Current Active, unrestricted state APRN license or ability to obtain prior to hire.
- Successful completion of an approved nurse practitioner program.
- Current approval by the Board of Nursing to perform medical acts.
- Master's degree of Nursing.
- Excellent organizational skills and attention to detail.
- Strong analytical and problem-solving skills.
- Able to stand and walk for 4+ hours
- Able to lift 100 pounds to assist fallen residents.
- Able to lift 10 pounds for routine performance of essential functions.
- Pushing/Pulling 70+ pounds when administering patient care
- Bending/squatting/kneeling/twisting when administering patient care
- Depth perception when performing medical procedures
- May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
- Will travel to multiple Skilled Nursing Facilities within your local area

How you make a difference
The Nurse Practitioner(NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarceratedpatientsin accordance withestablished policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director ordesigneetomonitorthe provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director andH.S.A. andmake arrangementsfor coverage of medical services if unavailable for an extendedperiod of time.
Key Responsibilities

Conducts routine and emergency medical care for inmates, including chronic care clinics, infirmary rounds, and emergency treatment on-site.

Adheres toestablishedpolicies and procedures for medical care, including documentation in SOAP format, adherence to formulary regimens, andutilizingavailable in-house resources for treatment.

Assumes responsibility and accountability for individual judgments and actions, while also reporting any concerns or questions to the Medical Director or Health Services Administrator.

Maintainspatient confidentiality and privacyin accordance withaccepted practices for incarcerated individuals.

Qualifications & Requirements
Education

A graduate of an accredited school of nursing

An advanced degree in an approved and accredited Nurse Practitioner program.
Experience

Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred.

Must have the patience and tact to deal with the inmatepatientpopulation and ability to work effectively in the corrections environment.

Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage.
Licenses/Certifications

Must have andmaintaincurrent licensure as a Nurse Practitioner within the state of employment.

Job Duties
May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. Meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
* Provides enhanced patient care under the direction of the licensed nurse, which may include but is not limited to advanced dressing care, suctioning, removal of Foley Catheter and peripheral IV.
* May perform venipunctures and skin punctures to obtain blood specimens according to Clinical Laboratory Standards Institute guidelines.
* May perform daily, hands-on aspects of the storage, preparation and delivery of breast milk, infant and pediatric formulas.
* Documents in the appropriate sections to create a complete and accurate medical record.
* Identifies, observes, and documents the physical needs of the patient according to established standards. Reports any changes in patient condition to licensed personnel in a timely manner.
* Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
* Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
* Provides one to one patient safety watch as assigned following established policies and procedures.
* Accurately processes specimens and provides timely transport of the specimen to the laboratory.
* Performs point of care patient testing and point of care instrument maintenance and quality control.
* Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
* Serves as a preceptor and mentor for new staff and students in a positive manner.
* Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard.
* Releases and confirms ordered tests to appropriate departments.
* Coordinate's transportation of patients to multiple tests.
* Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consult, etc.
* Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies.
* Communicates with food services to ensure appropriate patient diets.
* Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc.
* Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit.
* Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed.
* Assists with transporting blood products to and from Blood Bank as needed.
* Discharge follow up phone calls / coordinating virtual discharge appointment team.
* Support / Assist staff with 5 min Drills and/or Quality Rounds - participate in regulatory readiness as needed.

Full time position -36 hours/week
Hours: 7pm-7:30am (3-12 hour shifts/week)
Every other weekend and a holiday rotation
Experience preferred but willing to educate and train
Will need to attend 2 week daytime hours orientation program
Extra $2.00/hour float differential

Job Description
In collaboration with the physician, provides care to new or established patients. Demonstrates autonomy through patient evaluation, assessment, diagnosis, treatment and follow-up care.
