Physical Medicine and Rehabilitation (PM&R) or Physiatry is a branch of medicine that aims to enhance and restore functional ability and quality of life to those with physical impairments and disabilities, with a focus typically on the neuromusculoskeletal system. As an NP PM&R Clinician with Comprehensive Rehab Consultants (CRC), you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence. This position involves treating patients exclusively at a designated skilled nursing facility (SNF) or post-acute rehab center.

Responsibilities::
- Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
- Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside your PT, OT, and SLP teams to improve patient function and quality of life
- Follow therapy patients to track their progress in therapy
- Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!
- Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine
- Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues
The Ideal CRC Clinician will have:

Job Type: Full-time/Part-time
Pay:30-hour: 95,000 - 125,000 per year.
24-hour: Starting at 75.00/hr.

We are seeking Nurse Practitioners (NPs) or Physician Assistants (PAs) to join this Hospital-based cardiology practice, with a modern office located within the hospital. APPs are integral members of our department and clinical care teams that provide comprehensive care. In this position, the NPs/PAs work alongside an experienced cardiologist and medically manage a variety of cardiology outpatients.
Responsibilities:
- Onsite Cardiac Testing to include: EKG, Echocardiograms, Exercise Stress Echocardiogram, Dobutamine Stress Echocardiogram, Myocardial Imaging Spect Nuclear Stress Testing, Exercise Treadmill Stress Test, Ambulatory Blood Pressure Monitoring, E-Patch Holter Monitoring, Transthoracic Echocardiography, and Cardioversion.
- Within the Cardiology Office we manage a wide range of patients from Hypertension, Heart Failure, Heart Valve Defects, Cardiac Pre-Op Clearance, Post Hospital Discharge Cardiac Follow -up, ect. We also offer a full range of Implantable Devices Monitoring; in person or remote to include, however not limited to pacemaker and implantable defibrillators.
- Hospital services include: cardiac consultation and follow-up, pacemaker implantation, implantable loop recorders, EKG, Echocardiograms, Exercise Stress Echocardiogram, Dobutamine Stress Echocardiogram, and Myocardial Imaging Spect Nuclear Stress Testing.
- Call is not required for APPs
• Candidates must be board certified as a Nurse Practitioner or Physician Assistant and possess or be eligible to obtain an applicable New York State license.

We are seeking specialty trained Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our 1st assist and outpatient service teams. In this position, the NPs/PAs work alongside experienced physician and advanced practice providers (APPs) colleagues both surgically and medically managing a variety of orthopedic outpatients, adult and pediatric, and during and after their surgical procedures.
APPs are integral members of our department and clinical care teams that provide comprehensive care in a multitude of orthopedic subspecialties including general orthopedics, sports medicine, joint replacement, spine surgery, and foot and ankle surgery.
Responsibilities:
• Performing medical evaluations, treatment, follow-up and consultations for diverse orthopedic adult and pediatric traumas and acute care surgical patients.
• Medically triaging and managing outpatient orthopedic acute and chronic orthopedic conditions as well as occasional inpatient consults.
• Delivering direct patient care as part of a collaborative teaching service and patient care team.
• Gaining a working competency of procedures inclusive of, but not limited to, 1st assist and 2nd assist OR management, as well as, casting/splinting/traction and other routine orthopedic care interventions.

About the Position:
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

What to Expect :
- Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up
- Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits

Type: PRN - Flexible
PAY: Earn from $,300 up to 11,500 per month (Based on an average of 20-100 completed visits per month, and pay of 115.00 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF and other community environments in Ogdensburg , New York
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN providers are non-benefited except for 401(k) and limited Voluntary Benefits

Qualifications:Must Haves:
- Master's Degree required OR commensurate experience and satisfactory completion of NP licensure
- Master's Degree required OR commensurate experience and satisfactory completion of PA licensure
- Current RN and NP or PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority
- Board certified by the NCCPA, AANP, ANCC or the AACCN in a Matrix approved specialty
- Have a current BLS, ACLS or CPR certification
- 1 year experience as a Nurse Practitioner/Physician Assistant preferred, new grads encouraged to apply. Have the ability to travel either in state or out of state
- Have strong computer skills and familiarity with EMR software
- Possess excellent verbal and written communication skills with patient, clients and colleagues
- Have a positive and outgoing personality, and be a team player. Be comfortable and flexible with frequent change
Travel Requirements:
- Travel required to meet patients where they live
- Ability to travel - Valid state driver license, able to drive a car, proof of adequate automobile insurance coverage for the state of residence

The State University of New York (SUNY) college at Canton invites qualified applicants for Physician Assistant or Nurse Practitioner in the Student Health Center. This is an 11-month position.
Responsibilities
The role is responsible for providing assessment, diagnosis, appropriate medical management, and patient education for acute episodic illnesses, injuries, and stable chronic diseases in a college health center.
The PA/NP will engage in timely and appropriate consultation with the supervising physician and actively participate in health education and community outreach activities of the department as needed. Eleven-month permanent position available immediately with NYS benefit package.
Clinic hours are Monday through Friday, 8:00am-4:30pm. No nights or weekends.
Qualifications
Required:
- Candidates must be a graduate of an accredited educational program for Physician Assistants or Nurse Practitioners.
- Licensed to practice as a Physician Assistant or Nurse Practitioner in New York State.
- Experience working in a healthcare setting.
Prior to a final offer of employment, the selected candidate will be required to submit to a background check including, but not limited to, employment verification, educational and other credential verification, and criminal background check.
