*Highland Hospital is seeking a Full Time Family Nurse Practitioner to join its Highland Family Medicine Office, located on 777 S. Clinton Avenue, just around the corner from HH. Highland Family Medicine could entertain a Full Time, 36 or 40 hour a week employee for this role.
Some duties include but are not limited to:
Direct patient care, coordination of clinical services and assurance of continuity of care, resident and other student education, as well as assisting the coordination of team functions.
Provides direct patient care to patients from neonate through geriatrics according to community standards for clinical practice, Family Medicine protocols, procedures and guidelines, and professional Nurse Practitioner standards for advanced clinical practice.
Practices within the scope of New York State Nurse Practitioner regulations as per the New York State Nurse Practitioner Act as well as the Federal DEA prescriptive regulations if DEA registered.
Examines patients and prescribes treatment when indicated for individuals and families requiring acute and chronic health care, preventive and restorative care, and evaluation and care of physical, mental, and developmental function and capabilities.
**Salary Range:**
$98,000.00 - $130,083.20 annually

**Job Requirements:**
1). Education: Fulfills the educational requirements to be a licensed NP in NYS; specialization in Family Practice highly desirable; consideration given for nurse practitioners with other specialty training.
2). Experience: 3-5 years of experience as a registered nurse prior to completing a Master's program is desired. Minimum 1 year experience as a practicing Family Nurse Practitioner in an ambulatory primary care setting is desired. 3-5 years as an FNP in ambulatory care highly desirable. Bilingual and teaching experience is desired.
3). License/Certification Required: Licensure to practice as a nurse practitioner in the State of . Certification by the State of for specialty practice as a Family Nurse Practitioner with full prescriptive privileges. Registration by the Federal Drug Enforcement Agency to prescribe Category II through V drugs. Certification in Family Practice preferred from a national organization, such as, but not limited to, the of the American Nurses Association. This will be necessary for continued employment.
4). Skills: Ability to communicate effectively, both verbally and in writing. Proficient, efficient, and safe independent clinical practice capability. The Nurse Practitioner working primarily at Highland Family Planning must demonstrate competence with gynecological procedures including IUD insertions, inserting implantable contraceptive devices, administering injection contraceptives and immunizations as well as other skills necessary for the position within 1 month of hire.

**Responsibilities:**
GENERAL PURPOSE:
The School Based Health Center (SBHC) Family Nurse Practitioner (FNP)/Pediatric Nurse Practitioner (PNP) provides comprehensive adolescent health services to enrolled students, ages 10-21, in the school-based health center. The NP works in collaboration with the SBHC NP Supervisor/Medical Director. The Nurse Practitioner (NP) maintains a patient centered, customer-oriented approach to meeting patient needs and involving the family in the plan of care as appropriate. He/she maintains confidentiality and patient privacy. The NP works collaboratively with other SBHC providers and ancillary staff as well as community Primary Care Physician's (PCP's) and school personnel. The NP provides mental health assessments and medication management as clinically indicated. This is a 10-month position, September 1st to June 30th.
**ESSENTIAL FUNCTIONS**
+ Provides direct care through assessments, diagnosis, treatment planning, interventions and medication management, and management of acute illnesses and injuries, management of chronic conditions, health screenings and education, population-based primary prevention and monitoring outcomes of care.
+ Provides adolescent health services and counseling that include but not limited STI testing and treatment, emergency contraception (Plan B), Birth control, Pregnancy testing and counseling, other sexual health services.
+ Obtains and/or reviews/evaluates a patient's medical and psychosocial history and through patient evaluation and diagnostic studies, is able to determine a differential diagnosis and plan for intervention.
+ Provides medical services to patients in accordance with UR and SBHC clinical policies and procedures and documents care provided, meeting UR, DOH and Medicaid billing requirements.
+ Collaborates with appropriate members of the health team to facilitate the highest quality of services. Makes referrals to appropriate sources/consultants for medical and/or social problems.

**MINIMUM EDUCATION & EXPERIENCE**
+ Master's Degree from an Accredited Nurse Practitioner Program required.
+ More than 3600 hours of qualifying nurse practitioner practice experience required.
+ Experience working with adolescent population (clinical rotation &/or work), interest in adolescent mental health, comfortable working with a small team, collaborative, team player preferred.
**KNOWLEDGE, SKILLS AND ABILITIES**
+ Clinical Competence from very acute, chronic and well patients required
+ Fluency in Spanish language preferred.
**LICENSES AND CERTIFICATIONS**
