Job Summary

The Senior Clinical Informatics Specialist reports to the Associate CNIO and is directly responsible for providing consultation, coordination, and evaluation of clinical informatics processes and integration of those processes with system wide clinical information technology strategies.

Essential Functions
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Meets all standards of the Patient Satisfaction Initiative Criteria in order to make Jefferson Health the hospital of choice for Patients and Employees.
  • Works collaboratively to communicate and coordinate clinical operations improvement activities throughout assessment, implementation, and post go-live phases of clinical applications for an EMR.
  • Facilitates visionary, out-of-the-box thinking throughout workflow processes.
  • Implements processes for ongoing content management of best practice evidence-based workflows, order sets, forms, and other documentation tools that will meet minimum requirements and will be consistently applied throughout the system.
  • Participates in assessment of needs and implementation of strategies to link practice guidelines to practice-based electronic medical records systems, including recommendations on future direction for health care informatics within the organization.
  • Models teamwork within the department, and may function as a working lead if needed.
  • Regularly scans and reports on the body of nursing informatics literature and best practices.
  • Performs other duties as assigned.
Education:

BSN Required, Master s degree in relevant field preferred

AND

Experience:

2+ years nursing experience, 5+ years informatics or other related experience.

Competencies (Knowledge, Skills, and Abilities Required):

Knowledge of computer and database development.

Excellent interpersonal and communication skills, both written and verbal.

Ability to prioritize and function effectively with teams.

Proficiency in Microsoft Word, Excel, Power Point, Sharepoint, and Teams.

**How you make a difference**
The Nurse Practitioner(NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarceratedpatientsin accordance withestablished policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director ordesigneetomonitorthe provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director andH.S.A. andmake arrangementsfor coverage of medical services if unavailable for an extendedperiod of time.
**Key Responsibilities**
+ Conducts routine and emergency medical care for inmates, including chronic care clinics, infirmary rounds, and emergency treatment on-site.
+ Adheres toestablishedpolicies and procedures for medical care, including documentation in SOAP format, adherence to formulary regimens, andutilizingavailable in-house resources for treatment.
+ Assumes responsibility and accountability for individual judgments and actions, while also reporting any concerns or questions to the Medical Director or Health Services Administrator.

**Qualifications & Requirements**
Education
+ A graduate of an accredited school of nursing
+ An advanced degree in an approved and accredited Nurse Practitioner program.
Experience
+ Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred.
+ Must have the patience and tact to deal with the inmatepatientpopulation and ability to work effectively in the corrections environment.
+ Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage.
Licenses/Certifications
+ Must have andmaintaincurrent licensure as a Nurse Practitioner within the state of employment.

+ Obtain, record, and interpret complete health history, including past medical history, obstetrical history, family history and psychosocial aspects. Perform comprehensive physical assessment of the newborn/neonate, order and interpret laboratory, radiographic, and clinical data when planning and documenting the course of management discussed with the attending Neonatologist. **Overview**
We are looking for a **AIM Statewide Travel Nurse Practitioner** to join our team. This position will directly report to the AVP of Advanced Illness Management (AIM) and is responsible for facilitating and providing advanced illness management and palliative care services to palliative care patients.
+ Provide individualized treatment plans and comprehensive care from the comfort of patients' home: including providing diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director or Primary Care physician.
+ Role requires up to 80-90% statewide travel.
+ Requires regular travel between palliative branches throughout the state to provide patient care and other essential responsibilities.
+ May potentially be some occasional regional travel if needed (less than 10%).
+ Flex or alternative schedules available.
**About You**
+ Minimum of Master of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law, i.e. DNP (Doctorate of Nursing)
+ 2+ years Nurse Practitioner experience
+ Current license as a registered nurse and equivalent as required by state laws for Nurse Practitioner licensure without board actions.
+ Active and unrestricted Nurse Practitioner certification/license without board actions.
+ Multi state licensure is desirable for Travel Nurse Practitioner role or willing to obtain additional state licensure (company paid) for those interested in regional travel opportunities (e.g. assist with new site launches)
+ Practice experience with Part B documentation and coding
+ Must have a collaborative agreement in place with a supervising physician agreeable to supervise work in palliative care program or must agree to enter a collaborative agreement with Palliative Medical Director, as required by state law.
+ Part B Provider certification
+ Federal DEA license
+ State DEA license or state specific prescribing authority (if required)
+ ACHPN - Certification in Hospice and Palliative Care Preferred
+ Nurse Practitioner specialty in Family or Adult Medicine, Acute Care preferred
+ Travel: Up to 80-90% (by car)

Role Description
This resource is responsible for a broad range of health care services including medical assessment and prescription therapeutic measures, promotion and maintenance of health, prevention of illness and disability, care during phases of illness, guidance and counseling of individuals and families, and referral to other health care providers and community resources.
Work is performed under the direction of a licensed physician and is received in the form of oral and written instructions, standing orders, and protocols. Supervision of medically-related activities is received from a licensed physician who reviews work through conferences, records audits, and reports for effectiveness, compliance with standards, and adhere to Certified Registered Nurse Practitioners regulations and advanced nursing practices. General Supervision is received from a facility administrator.
