In this role, you will provide high-quality healthcare services to patients, working closely with our multidisciplinary team to ensure that each individual receives the care they need. Duties and responsibilities

  • Provide direct patient care including assessments, diagnoses, treatments, and follow-up care.
  • Perform comprehensive physical examinations and health screenings.
  • Order, interpret, and act on diagnostic tests and laboratory results.
  • Develop treatment plans in collaboration with patients and their families, ensuring individualized care.
  • Prescribe medications and therapies as appropriate.
  • Educate patients on health promotion, disease prevention, and management of chronic conditions.
  • Provide guidance and support for patients navigating gender identity and sexual health concerns.
  • Collaborate with a team of healthcare providers including physicians, mental health specialists, and social workers.

  • Advocate for patients needs and coordinate care among various healthcare providers.
  • Prescribes, adjusts, and monitors medications, ensuring appropriate dosages and minimizing side effects. Educates patients on medication use and potential risks.
  • Takes a patient-centered approach to develop individualized treatment plans in collaboration with patients, considering their unique needs, preferences, and goals.
  • Understands legal and professional responsibility of "duty to report" and proceeds according to Florida state law associated with child and elder abuse.
  • Maintains accurate and up-to-date electronic medical records, including progress notes, treatment plans, and medication records, in compliance with all legal and ethical standards.
  • Maintains all required licenses, board certifications, registrations, and any other required qualifications or credentials, keeping them up to date and in compliance prior to any expiration dates.
  • Completes assigned annual company trainings for compliance.
  • Stays current with the latest developments in the field through ongoing professional development and training.
  • Adheres to clinical guidelines and best practices in patient care.
  • Participates in quality improvement initiatives and contributes to the clinic's commitment to excellence in healthcare delivery.
  • Attends outreach events as requested.
  • Performs other duties as required.
Qualifications/Knowledge/Skills/Experience
  • Valid WHNP, APRN or NP license in Florida, as well as DEA number.
  • Bachelor's degree in nursing
  • Minimum of 3 years of clinical experience in a healthcare setting.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a collaborative healthcare team.
  • Empathy and compassion for individuals with health challenges.
  • Commitment to maintaining patient confidentiality and adhering to ethical standards.
  • Proficiency in electronic health records (EHR) systems.
  • Commitment to patient-centered care.
  • Telemedicine experience is a plus but not required.

Position Overview:

We are seeking a highly skilled and adaptable Nurse Practitioner to join our multidisciplinary healthcare team. This hybrid role spans multiple lines of business, providing services across Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and Transitional Care Management (TCM) for in-home and telemedicine patients. The NP will be trained in Osteopathic Manipulative Treatment (OMT) follow-up care in SNFs, oversee pain management in ALFs, and serve as the primary provider for TCM patients, ensuring comprehensive care coordination and improved patient outcomes.

Key Responsibilities:

SNF (OMT Follow-Up)

• Collaborate with Osteopathic Physicians to deliver OMT follow-up care for post-treatment evaluations.

• Monitor patient progress, document clinical outcomes, and adjust care plans as directed.

• Communicate effectively with SNF staff, therapy teams, and families regarding patient progress.

ALF (Pain Management)

• Conduct pain assessments and develop individualized pain management plans in coordination with facility staff.

• Serve as a liaison between residents, facility teams, and attending physicians to ensure quality pain management care.

Qualifications:

• DEA registration and prescriptive authority.

• Minimum 2 years of clinical NP experience; post-acute or geriatric experience preferred.

This position is responsible for performing home visits to manage the patient's wound care, as well as documenting their visits in a timely and efficient manner.

RESPONSIBILITIES:
• Provision of hands-on care to patients as directed by Woundtech's needs.
• Debridement of wounds by sharp, enzymatic, autolytic or surgical methods completed safely during the patient visit.
• Application of topical wound care and associated wound management products.
• Recommendation of associated services to improve wound healing outcomes. This includes, but is not
limited to, labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
• Interpretation of diagnostic tests and recommendation of appropriate follow up to same.
• Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
• Risk assessment for Members with Diabetes, as per the ADA guidelines.
• Patient education and management.
• Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
• Provide patient updates to PCP, CM, and Physicians.

REQUIREMENTS:
• Graduate nursing degree from accredited school of Nursing and Nurse Practitioner program or Physician Assistant graduate from an accredited school for Physician Assistant education.
• Possess and maintain national certification as NP or PA; wound care certification preferred.
• 1-3 years' experience at the advanced practice level.
• Current, active, unrestricted license as a Nurse Practitioner or Physician Assistant in the state.
• Ability to practice autonomously.
• Ability to lift 50 lbs. or more.
• Must have full range of mobility.
• Must be able to reposition patients.
• Valid and unrestricted driver's license.
• Excellent verbal and written communication skills in a professional environment.
• Ability to set priorities while managing multiple tasks.
• Detail oriented with excellent organization skills.
