**Overview**
We're looking for a **Hospice Nurse Practitioner** to join our team.You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
+ Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
+ Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
+ Makes appropriate recommendations to the IDG based on findings of the F2F assessment
+ Notifies hospice agency when unable to complete a F2F visit according to schedule
**About You**
+ Master's or higher degree in Nursing.
+ Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
+ Current state license as a registered nurse
+ Current state license or certificate as an advanced practice nurse
+ Current national certification as a Nurse Practitioner. (ANCC or AANP)
+ Must have a collaborative agreement in place.

**Patient Care** :
· Evaluates and treats center patients in accordance with standards of care?
· Follows level of medical care and quality for patients and monitors care using available data and chart reviews?
· Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care?
· Acts as an active participant and lead with the care team through daily huddles.
· Helps Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
· Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity?
· Follows policy and protocol defined by Clinical Leadership?
· Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues?
· Participates in potential growth opportunities for new or existing services within the Center?
· Participates in the local primary care "on-call" program of CenterWell Senior Primary Care?
· Assures personal compliance with licensing, certification, and accrediting bodies?

**Required Qualifications:**
+ Minimum of two to five years directly applicable experience preferred?
+ Successful completion of a Nurse Practitioneror Physician Assistant program with Board Certification?

+ Excellent communication skills; written and verbal, demonstrate a high level of skill with interpersonal relationships and communications with all colleagues; fully engaged in the concept of "Integrated team based care"?

Job Summary and Qualifications

The Patient Care Technician (PCT) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living. Under the supervision of the registered nurse (RN), the PCT assists the nursing staff in providing patient care in accordance with the PCTs skills/training and the needs of the Department, and consistently assists the patient care team with maintaining a clean, well-organized, and safe patient care environment. The PCT complies with all hospital policies/procedures and observes the State Board of Nursing rules for unlicensed personnel.

What you will do in this role:

  • Participates in the implementation of nursing care plans by performing direct and indirect patient care under the supervision of the licensed nurse.
  • Obtains and records initial and ongoing patient information throughout the cycle of care, including admissions data, vital signs, height and weight, blood pressure, oxygen saturation, intake and output, calorie counts, and other patient information as outlined by policy.
  • Assists with mobility by regular turning and positioning, dangling, and ambulating patients. Assists with use of walkers, crutches, canes, and wheelchairs. Transfers patients to and from other units, departments, or entities. Assists with discharges. Demonstrates proper body mechanics.
  • Obtains or assists in the collection of specimens for diagnostic or waived tests. Ensures accuracy of all labels and timely transfer of specimen.
  • Assists patients with activities of daily living to include eating and drinking, bathing, shaving, oral care, grooming, and elimination. Changes bed linens as needed.
  • Serves as Monitor/Telemetry Tech for the unit as needed.
  • Engages in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.
What qualifications you will need:
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date

**Job Summary and Qualifications**
The Patient Care Technician (PCT) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living. Under the supervision of the registered nurse (RN), the PCT assists the nursing staff in providing patient care in accordance with the PCT's skills/training and the needs of the Department, and consistently assists the patient care team with maintaining a clean, well-organized, and safe patient care environment. The PCT complies with all hospital policies/procedures and observes the State Board of Nursing rules for unlicensed personnel.
**What you will do in this role:**
+ Participates in the implementation of nursing care plans by performing direct and indirect patient care under the supervision of the licensed nurse.
+ Obtains and records initial and ongoing patient information throughout the cycle of care, including admissions data, vital signs, height and weight, blood pressure, oxygen saturation, intake and output, calorie counts, and other patient information as outlined by policy.
+ Assists with mobility by regular turning and positioning, dangling, and ambulating patients. Assists with use of walkers, crutches, canes, and wheelchairs. Transfers patients to and from other units, departments, or entities. Assists with discharges. Demonstrates proper body mechanics.
+ Obtains or assists in the collection of specimens for diagnostic or waived tests. Ensures accuracy of all labels and timely transfer of specimen.
+ Assists patients with activities of daily living to include eating and drinking, bathing, shaving, oral care, grooming, and elimination. Changes bed linens as needed.
+ Serves as Monitor/Telemetry Tech for the unit as needed.
+ Engages in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.

**Overview**
We're looking for a **Patient Care Manager Senior** to join our team. You will report directly to the Executive Director or Administrator. You will be responsible for assisting branch leadership with maintaining a high performing hospice program including managing, overseeing, and directing patient care activities to ensure the delivery of quality patient care. This role is responsible for supervising, teaching, evaluating, and developing associates and ensuring adherence to company policies and procedures and compliance with local, state, and federal regulations and regulatory agencies.
+ Preparing for, serving on, and facilitating the Interdisciplinary Group (IDG), as assigned, including presiding over IDG, presentation of patients for review, coordination of minutes, review of patient charts to note and address any changes to patient condition and/or the plan of care, and soliciting input from various care team members.
+ Managing and overseeing clinical and administrative and recordkeeping functions, including documenting referrals, new physicians and facilities, durable medical equipment (DME) pickup, Medicare patient eligibility, obtaining authorization and reauthorization as needed, commercial insurance verification, transfer patient documentation, satisfaction surveys, and patient benefit periods and events.
+ Directing the work of clinical associates including assigning of admit and evaluation visits, scheduling visits at the beginning of benefit period, and ensuring staff are completing visits in a timely manner.
**About You**
+ Graduate of accredited school of nursing; bachelor's degree in nursing preferred
+ Current Registered Nurse licensure in state of residence and requested employment
+ At least three years' experience of direct patient care and two years' experience in hospice or home-based care setting.
+ Demonstrates knowledge and compliance with accepted hospice principles and practice, including industry standards, regulations, and best practices (i.e., Medicare, Medicaid, JCAHO, ACHC), company policies/procedures, and understanding of terminally ill patients and their families.
+ Thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, JCAHO, ACHC, and human resource) management principles
+ Ability to provide program management and oversight, including documentation to ensure quality, compliance, and accuracy

Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
Education/Communication:
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
**INFECTION CONTROL:**
+ Assists in collecting information for infection control audits.
+ Supports staff and patient Adherence to infection control practices.
+ Follows infection control Policies and Procedures
+ Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.
**TRANSITIONAL CARE UNIT:**
+ Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care
+ Reinforces all education and care related matters as it relates to the new patient as allowed by state law
+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.
+ Assists with supply ordering and inventory for TCU
