The **Palliative Care Nurse Practitioner** facilitates palliative care for Gentiva Palliative Care patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. **As a Nurse Practitioner, you will:**
+ Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.
+ Take responsibility for patient palliative care from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.
+ Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team.
+ Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.
+ Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
+ Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.
+ Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.
+ Maintain effective working relationships with supportive care team (e.g., Social Worker, Spiritual Care Coordinator), as applicable, and involve team when patient's care plan warrants team participation to achieve desired outcome.
+ Establish and maintain effective working relationships with those contacted in the course of work.
+ Communicate and document clinical findings, treatment plan and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.
+ Knowledgeable of Palliative Medicine Quality Measures collection, documentation and reporting.

+ Excellent leadership, interpersonal, written and verbal communication skills.
+ Independent decision making and self motivation.
+ Working knowledge or understanding of: principles of Palliative Medicine, including when a patient may be appropriate for home health or hospice care; Medicare/Medicaid regulations; and interdisciplinary process.
+ Meets mandatory continuing education requirements of Gentiva and licensing board.
+ Must be able to work efficiently with point of care software.
**Education/Experience:**
+ Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law

+ Practice experience with Part B billing

PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.
- Sets up, tests, and operates hemodialysis machines for patient treatments.
- Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
- Evaluates vascular access pre-treatment and performs vascular access cannulation.
- Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
- Monitors patients' response to dialysis therapy.
- Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
- Reports any significant information and/or change in patient condition directly to supervisor.
- Enters all treatment data into the designated clinical application in an accurate and timely manner.
- Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
- Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
- Assists other healthcare members in providing patient education.
- Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
EDUCATION AND LICENSES:
- High school diploma or G.E.D. required.
EXPERIENCE AND REQUIRED SKILLS:
- Qualities and traits: dependable, compassionate, caring, supportive, collaborative, reliable.
- Previous patient care experience in a hospital setting or a related facility (preferred but not required).
- Continued employment is dependent on successful completion of the Fresenius Medical Care dialysis training program and successful completion of CPR certification.

Overview:
Under the direction of the RN or LPN, assists in the individualized care of the patient to achieve the patient's highest level of wellness. Works cooperatively with others as part of a team; recognizes the importance of group goals. Performs designated clinical procedures and non-clinical support tasks essential to providing care to the patient.
About our Surgical Unit:
Surgical floor at Mercy Jefferson provides comprehensive, interdisciplinary care to adult surgical patients who require post op surgical care. Our patients include but are not limited to:- General surgery
- Bariatric surgery
- Total joint replacement
- Fracture repair
- Urological Procedures
Qualifications:
- Education : High school diploma, G.E.D., CNA/CPCT or enrolled in a healthcare career program through their high school curriculum, and/or a current co-worker of Mercy.
- Experience : None required- In the state of practice: If no previous experience, Mercy will provide a required 175-hour training program that meets the state of practice Department of Health and Senior Services requirements. The program will be completed as part of the new hire orientation process and must be completed within 90 days of hire to remain in the PCT position per state guidelines.
- Certifications: BLS Required within one month of hire, completion of PCT competencies within 3 months of hire.
- Other : Minimum Physical Requirements- Position requires individuals to push, pull, and/or lift 50 pounds on a regular basis.- Position requires prolonged standing and walking each shift.- Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties.
- Preferred Experience : EMT, Paramedic or Previous Patient Care Experience.- Preferred Certifications: Certified Nursing Assistant (CNA), Emergency Medical Technician (EMT), Advanced Unlicensed Assistant (AUA) in the state of practice, Certified Patient Care Technician (CPCT).
