Physical Medicine and Rehabilitation (PM&R) or Physiatry is a branch of medicine that aims to enhance and restore functional ability and quality of life to those with physical impairments and disabilities, with a focus typically on the neuromusculoskeletal system. As an NP/PA PM&R Clinician with Comprehensive Rehab Consultants (CRC), you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence. Responsibilities:
- Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
- Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside your PT, OT, and SLP teams to improve patient function and quality of life
- Follow therapy patients to track their progress in therapy
- Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!
- Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine
- Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues

Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time Nurse Practitioner. The Nurse Practitioner position serves as a direct care provider for behavioral health services and primary care for clients (ASAM III.5). Clients in this program require medical assessment and oversite during the course of their treatment.
Responsibilities and Duties
It is the duty and responsibility of the Nurse Practitioner to do the following:
- provide direct care to patients within the Scope of Practice for which the provider is licensed;
- manages patient care under Best Practices, UDS recommendations, and as prescribed to maintain accreditation;
- conduct thorough documentation of visits and medical orders to support billing charges;
- complete billing components of visit to optimize revenue within dictated time frames;
- oversee care delivered by medical support staff by providing feedback on immediate care activities and reporting to direct supervisor on concerns that expose the agency or endanger a patient's health outcomes;
- participate in webinars and conference calls relative to assigned program;
- implement and manage Best Practices initiated by Quality Improvement/Quality Assurance staff;
- identify and prioritize medical decision making with a fiscally minded approach to care;
- coordinate and supervise the completion of daily tasks related to direct patient care on matters related to Health Outcomes i.e. referrals, lab orders, lab review, medical records review;
- organize and implement work flow strategies to support provider visits, billing and patient care as part of feedback to Program Manager during supervision;
- coordinate with Quality Improvement Program Manager in implementation of Quality Assurance measures to ensure data integrity and outcomes are met;
- assure support staff document all pertinent health related information in the client's electronic medical chart as instructed and as part of standards of practice designed to improve patient care, support billing, and meet Quality Improvement goals;
- observe Occupational Safety and Health Administration during all elements of daily operations, including but not limited to phlebotomy, in-house testing, medical interventions, medical waste and managing infection control;
- observe Health Insurance Portability and Accountability Act compliance; and
- perform other duties as assigned.
It is the duty and responsibility of the Nurse Practitioner to:
- meet regularly for One on One meetings with the Program Manager;
- participate in regularly scheduled Program meetings;
- respond to the Chief Medical Officer as needed;
- ensure timely completion of all assigned administrative reports;
- keep licensure current and meet credential requirements as requested;
- meet with the Chief Medical Officer on the practice and update as needed on trends, changes, alerts, concerns, and any matter which impacts patient care, services rendered, or billing when asked to do so;
- attend continuing education programs that provide updated information on health topics pertinent to caring for patients and effective clinic practice;
- participate in design, implementation, and support of Quality Improvement/Quality Assurance initiatives within the setting; and

+ Must have computer skills and dexterity required for data entry and retrieval of information.

**Job Duties**
+ Utilizes the approved medical practices process in the delivery of all patient care.
+ Provides care based on physical, psycho/social, educational, safety and related criteria, appropriate to the ages of patients served in assigned areas.
+ Maintains an attitude of inquiry towards medical practices and patient care outcomes and may develop research methodology.
+ Provides education experiences to patient families, staff, other health professionals, and to the community.
+ Performs duties including but not limited to ordering and in interpreting radiology tests, Point of Care Testing and lab testing, EKG reading, minor procedures (lacerations, incision and drainage abscesses), and pediatric population (6 month and older).
+ Provides training and education to staff, including new APPs and students.
+ Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
+ Other related duties as required.

Duties performed routinely require exposure to blood, body fluid and tissue.
The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
