**Seeking Nurse Practitioners licensed in TWO or more of the following states: MA, VA, TN, FL, IN, OH, TX, KS, AZ, IA, NM, NJ, HI and must have an active RN Compact license.**
**Location: Virtual** - This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

**Work Shift: 11am - 11pm CST; The NP will work six (6) 12-hour shifts and one (1) 8-hour shift in a two-week period, including Saturday and Sunday every other weekend.**
The **Advanced Practice Provider** is responsible for collaborating with company physicians, the patient's other physicians and providers, and their family members to develop complex plans of care in accordance with the patient's health status and overall goals and values. Provides clinical and non-clinical support to patients.
Primary duties may include but are not limited to:
+ Provides primary and urgent health care via telephone and tele video modalities to patients who receive home and community-based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.
+ Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions.).
+ Gathers history and physical exam and diagnostics as needed and then develops and implements treatment plans given the patient's goals of care and current conditions. Identifies and closes gaps in care.
+ Meets the patient's and family's physical and psychosocial needs with support and input from the company's inter-disciplinary team.
+ Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.
+ Maintains contact with other clinical team members, patients' other physicians and patients' other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis and consistent with state regulations and company health standards and policy.
+ Maintains patient medical records and medical documentation consistent with state regulations and company standards and policy.
+ Participates in continuing education as required by state and certifying body.
+ Prescribes medication as permitted by state prescribing authority.
Position Requirements:
+ Requires an MS in Nursing.
+ Requires an active national NP certification.
+ Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in applicable states.
+ Experience working with Electronic Medical Records (EMR) required.
+ Requires 2+ years of experience in managing complex care cases.
Preferred qualifications, skills and experiences:
+ Emergency Room and/or Urgent Care experience is strongly preferred.

We are seeking a Nurse Practitioner at our Louisville, KY clinic to help ensure our service members get the best care possible. You will play a vital role in the continued care of these men and women.
Your Role at Jamshid Tamiry, M.D., P.C., dba QTC Medical Group of Alaska:
As a Nurse Practitioner with Jamshid Tamiry, M.D., P.C., dba QTC Medical Group of Alaska, a Leidos QTC Health Service affiliated professional corporation, you will conduct a variety of general medicine, occupational, and disability health examinations including, but not limited to:
- Reviewing medical history and associated records
- Interpreting clinical data
- Completing written reports and generating independent medical opinions (IMO)
The examinations may be completed in-person (in a clinic setting), in the examinee’s home, telephonically, at large event settings for the Reserve Health Readiness Program, or using telehealth platforms.
You will be traveling (up to 30%) to provide physical examinations for veterans and service members in multiple locations throughout the country. All travel expenses are covered and a per diem will be received.

Essential Duties and Responsibilities:
- Physical exams for a variety of customers to include federal, defense and others.
- Occupational health exams to include pre- and post-employment and annual physicals.
- Written medical opinions based on provided medical records/information and acceptable clinical evidence (ACE)
- Provide telephonic and virtual exams
- Unique exam settings - homebound, event sites, etc.
- Completion of Veterans Benefits Administration Disability Benefit Questionnaires (DBQs)
- Provide primary medical evaluations to include, but not limited to: review of systems, pap smear, and range of motion
- Complete initial review and interpretation of diagnostic studies to include but not limited to: laboratory, pulmonary function and ECG studies
- Documentation of examinee records in appropriate systems.
- Collaboration with other professional and clinical staff as needed and communicate with internal and client team members in determining the most effective and evidence-based way forward on behalf of the examinee population.

We are seeking a Nurse Practitioner at our Louisville, KY clinic to help ensure our service members get the best care possible. You will play a vital role in the continued care of these men and women.
Your Role at Jamshid Tamiry, M.D., P.C., dba QTC Medical Group of Alaska:
As a Nurse Practitioner with Jamshid Tamiry, M.D., P.C., dba QTC Medical Group of Alaska, a Leidos QTC Health Service affiliated professional corporation, you will conduct a variety of general medicine, occupational, and disability health examinations including, but not limited to:
- Reviewing medical history and associated records
- Interpreting clinical data
- Completing written reports and generating independent medical opinions (IMO)
The examinations may be completed in-person (in a clinic setting), in the examinee’s home, telephonically, at large event settings for the Reserve Health Readiness Program, or using telehealth platforms.
You will be traveling (up to 30%) to provide physical examinations for veterans and service members in multiple locations throughout the country. All travel expenses are covered and a per diem will be received.
Work Schedule, Location, What to expect:
- Generally M-F, working an 8 hour day (between 7am -6pm), with weekend/evening schedules based on operational needs.
- 30% national travel

Essential Duties and Responsibilities:
- Physical exams for a variety of customers to include federal, defense and others.
- Occupational health exams to include pre- and post-employment and annual physicals.
- Written medical opinions based on provided medical records/information and acceptable clinical evidence (ACE)
- Provide telephonic and virtual exams
- Unique exam settings - homebound, event sites, etc.
- Completion of Veterans Benefits Administration Disability Benefit Questionnaires (DBQs)
- Provide primary medical evaluations to include, but not limited to: review of systems, pap smear, and range of motion
- Complete initial review and interpretation of diagnostic studies to include but not limited to: laboratory, pulmonary function and ECG studies
- Documentation of examinee records in appropriate systems.
- Collaboration with other professional and clinical staff as needed and communicate with internal and client team members in determining the most effective and evidence-based way forward on behalf of the examinee population.

- From a primary care perspective, be able to fully assess examinee health status through physical examinations.

- Technically competent with computers/tablets to include proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and using Electronic Medical Records

Required Education and/or Experience: (includes certificate & licenses)
- Masters of Science in Nursing (MSN) from an accredited college or university.
- Current National Licensing w/ ANCC or AANP
- Minimum 12 years of post-graduate primary care / internal medicine experience.
- Unrestricted State medical licensure without limitations to perform full scope of authorized practice in the state of hire.
- Ability to obtain DEA license
- Valid BLS OR ACLS certification
- Ability to earn and maintain clinical training/certifications as required by current and future contracts.
- Must be able to obtain and maintain government clearances (LARS, Public Trust, NACI, etc.) as required.
- Ability to pass a medical clearance consisting of passing an N95 respirator fit test, properly wearing organizational standard respiratory protective equipment as required, passing a Tuberculosis/TB test, and receiving a Hepatitis A/B vaccine series or passing a Hepatitis A/B Titer test and other medical clearances/vaccinations as required.
Preferred Experience:
- Experience conducting occupational/disability medical examinations.
