Seeking Bilingual Spanish Nurse Practitioners licensed in TWO OR MORE of the following states: MA, VA, TN, FL, IN, OH, TX, KS, AZ, IA, NM, NJ, or HI and must have an active RN Compact license.
Location: Virtual - This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.
Work Shift: 11am - 11pm CST; The NP will work six (6) 12-hour shifts and one (1) 8-hour shift in a two-week period which includes Saturday and Sunday every other weekend.
The Advanced Practice Provider is responsible for collaborating with company physicians, the patient's other physicians and providers, and their family members to develop complex plans of care in accordance with the patient's health status and overall goals and values. Provides clinical and non-clinical support to patients.
Primary duties may include but are not limited to:

Provides primary and urgent health care via telephone and tele video modalities to patients who receive home and community-based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.

Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions.).

Gathers history and physical exam and diagnostics as needed and then develops and implements treatment plans given the patient's goals of care and current conditions. Identifies and closes gaps in care.

Meets the patient's and family's physical and psychosocial needs with support and input from the company's inter-disciplinary team.

Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.

Maintains contact with other clinical team members, patients' other physicians and patients' other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis and consistent with state regulations and company health standards and policy.

Maintains patient medical records and medical documentation consistent with state regulations and company standards and policy.

Participates in continuing education as required by state and certifying body.

Prescribes medication as permitted by state prescribing authority.
Position Requirements:

Position Description: Circadia Health is launching a new, proactive model of care aimed at improving outcomes through early intervention and consistent monitoring in skilled nursing facilities (SNFs). As part of this Proactive Care Program, we are seeking a dedicated Nurse Practitioner (NP) to lead on-the-ground efforts across two partner SNFs in New Jersey, located 13 miles apart.
The NP will play a critical role working with the entire facility's patient population (short and long term). This includes identifying and managing patients at risk - including new admissions, patients with out-of-range vitals (e.g., blood pressure, blood sugar, weight), those flagged by facility staff, Circadia's AI Predictive Model, and/or patients with behavioral health needs.
You will play a crucial role in our mission to enhance patient care through innovative solutions. You will be responsible for performing in-person patient assessments, prescribing necessary medication and treatments, developing individualized care plans and collaborating with the interdisciplinary team to ensure the highest standards of care.
Key Responsibilities
- Proactive In-Facility Rounding : Conduct initial and routine assessments on patients, prioritizing those flagged by Circadia's AI powered monitoring system or facility staff. Prescribe all necessary medications and treatments under the covering physician agreement.
- Data - Driven Patient Identification : Review alerts and trend analyses generated by Circadia's platform to identify early signs of clinical deterioration. Verify and act on facility-initiated notifications within agreed response times.
- Personalized Care Planning : Develop and update patient-specific care plans that integrate AI insights, evidence-based guidelines and resident goals. Set and monitor threshold-based interventions (e.g. BP, glucose, weight parameters).
- Interdisciplinary Collaboration & Education : Coordinate with facility clinical teams to ensure seamless hand-offs and follow-through on escalations and interventions. Communicate with physicians and/or specialists regarding patient care. Provide education to facility staff on Circadia's platform and AI insights.
- Continuous Quality Improvement: Track key performance indicators (e.g. hospital transfers, response times, guideline adherence) and participate in regular QAPI meetings to drive outcome improvement.

Required Qualifications:
- Licensure: You must hold a current, unrestricted license as a Nurse Practitioner in the state where this job will be performed. If you do not currently hold a license, you must be in the process of obtaining one.
- Education: A Master's degree is required from an accredited Nurse Practitioner program.
- Certification: Must be BC by the AANP or ANCC.
- CPR Certification: You must have current CPR certification, or be willing to obtain it prior to your start date.
- DOT Certification: You need to have DOT certification, or be able to obtain it within the first 30 days of employment.

The NP functions independently and collaboratively to diagnose, treat, and manage acute and chronic conditions within the scope of HIV primary care.

Scheduling Requirements:
- Per Diem position hours based on clinic needs, with flexibility required
Essential Functions:

Provide outpatient primary and HIV specialty care, including physical exams, diagnostic workups, treatment plans, and follow-up

Manage antiretroviral therapy, PrEP/PEP protocols, STI screening and treatment, and co-morbid conditions

Collaborate with interdisciplinary care team to coordinate care and address social determinants of health

Educate patients and their families on disease processes, treatment options, medication adherence, and preventive health strategies

Maintain accurate and timely documentation in the EMR, ensuring compliance with HRSA Ryan White program standards and clinical guidelines

Participate in case conferences, quality improvement initiatives, and program development activities as needed

Provide trauma-informed and inclusive care to diverse populations, including LGBTQ+ individuals, undocumented immigrants, and persons with substance use and mental health concerns
