Job Summary:
Under the direction of the Medical Director and Overseeing Physician, the Nurse Practitioner manages the day-to-day operations of the onsite health care clinic. In addition, the Nurse Practitioner provides patient care that includes, but not limited to, case management, physicals and clearance exams, injury assessment, and minor procedures.
Essential Duties and Responsibilities:
- Perform post-offer physicals and non-work-related/return-to-work medical clearance examinations
- Provide urgent and episodic care to employees
- Respond to in-plant medical emergencies and evaluate employees with acute episodic illness, referring when appropriate
- Assess, diagnose, treat, and provide ongoing medical management of workplace injuries and non-occupational conditions (episodic and urgent), including writing prescriptions and facilitating referrals in collaboration with the physician
- Drive chronic disease management process, including education and wellness physicals
- Maintenance of charts and employee incident records
- Perform minor procedures including, but not limited to, suturing, splinting, joint injection, trigger finger injection, arthrocentesis, and x-ray interpretation
- Case management of workman’s compensation claims in conjunction with the employer representative and insurance carrier
- Facilitate return-to-work and light-duty assignments for employees with work-related injuries
- Maintain current case log for workplace injuries outlining treatment plan for weekly case review
- Assure compliance with OSHA and DOT programs such as hearing conservation, blood-borne pathogen training, random drug screening, and follow-up programs

Required Education, Skills, and Abilities:
- 3+ years of experience as a Nurse Practitioner
- Experience in an occupational health setting
- Bachelor’s degree in related field
- Current valid state licensure

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our De Leon and Carrollwood clinics and home-based care in the greater De Leon/Carrollwood area. Job Description
The Nurse Practitioner will act as part of a care team that directly interfaces with a small panel of patients with CKD stages 3b, 4, 5, and ESRD in order to drive high quality value-based care. He/she will interact with and evaluate patients in the home, via teleconference, or via telephone as appropriate. Job Summary
Administer 9 non-diagnostic annual screenings per day, Monday through Friday, to public safety personnel. Must be able to work with a team as well as interact professionally with patients and client's Head of Department. Expected to work full time to achieve top of the line physicals as well as deliver screening results to patient with follow-up guidance.
*This position requires frequent expense-paid travel for one week at a time.
Requirements
Education
- Master's degree (MSN) required
- Certified as a Family or Adult Nurse Practitioner.
- At least 3 years experience as an RN or 3+ years experience as APRN
- Licensed as a Registered Nurse and Nurse Practitioner in the state of practice
- BLS Required
- ACLS Required
Must be able to provide a general health exam. Should be comfortable with giving all general lab work results and guidance for abnormalities. Should be familiar with general cardiology and pulmonology. Expected to work closely with Ultrasound Sonographer and Exercise Physiologist to deliver ultrasound organ and cardiac/pulmonary findings to the patient and department heads when applicable.

Job Summary
Under the direction of a licensed physician, provides medical services to patient using approved patient care protocols; collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and professional consultations and provides direct patient care services. Formulates and implements treatment and therapeutic plans for hospitalized patients.
Duties & Responsibilities
Essential Duties & Responsibilities
- Provides comprehensive clinical primary patient care through interviewing, examining and establishing treating plan for patients to meet their health and wellness needs.
- Prescribes appropriate medical treatment and clinical medications consistent with Medical Standards for patients and orders studies, tests, and ancillary care services as required
- Creates medically appropriate referrals
- Follows all established rules and guidelines of evidence-based care and best clinical practice
- Documents all services in electronic patient medical record
- Regularly reviews chronic medical conditions that require ongoing medical attention on office visits.
- Educates patients on wellness, prevention, and early detection by discussion and /or providing materials and resources to the patients and families
- Provides routine office injections SQ, IM, or Intraarticular as per standards of care for primary care providers and Cano Health care standards.
- Consults/collaborates with Cano Health Physician as needed regarding patient care, assessment, and education issues in patients with complicated medical conditions.
- Maintains privacy and confidentiality of all patient information according to federal and state guidelines, regulations and HIPAA.
