Position Overview

The Licensed Vocational Nurse (LVN) oversees nursing assistants and ensures quality patient care in compliance with facility policies, regulations, and best practices. This role involves administrative duties, direct patient care, medication administration, and collaboration with healthcare teams to maintain high standards of resident well-being.

Key Responsibilities

Administrative Duties

  • Supervise daily activities of nursing assistants in accordance with facility guidelines.
  • Ensure compliance with written policies and procedures.
  • Review and recommend updates to department policies and manuals.
  • Plan and oversee shift services, programs, and activities.
  • Complete required reports, records, evaluations, and documentation.
  • Participate in quality assurance programs and facility inspections.
  • Assist with resident admissions, transfers, and discharges.

Charting & Documentation
  • Maintain accurate and up-to-date resident records.
  • Document physician orders, medication administration, and resident progress.
  • Report and record incidents, accidents, and care discrepancies.
  • Ensure compliance with charting and documentation protocols.

Medication Administration
  • Prepare and administer prescribed medications safely and accurately.
  • Verify resident identity before administering treatments.
  • Maintain accurate narcotic records and report discrepancies.
  • Ensure proper handling and disposal of medications per regulations.

Personnel Management
  • Assist in staff evaluations, scheduling, and performance oversight.
  • Ensure nursing assistants meet training and certification requirements.
  • Provide leadership, training, and support to assigned staff.
  • Enforce facility policies, procedures, and safety regulations.

Resident Care & Support
  • Assist with resident admissions and orientation.
  • Coordinate with physicians for treatment plans and medical needs.
  • Monitor residents' physical and emotional well-being.
  • Provide direct nursing care, including wound care, catheterization, and therapy services.
  • Respond to resident and family concerns professionally and compassionately.

Safety & Compliance
  • Ensure adherence to infection control protocols and safety standards.
  • Participate in emergency response and disaster preparedness.
  • Monitor equipment use and report maintenance needs.
  • Follow state and federal healthcare regulations.

Qualifications

  • Education: Graduate of an accredited LVN program or equivalent.
  • Licensure: Active, unencumbered LVN license in CA.
  • Certifications: Current CPR certification; PCC experience preferred.
  • Skills: Strong leadership, problem-solving, and communication abilities.
  • Physical Requirements: Ability to lift 25 lbs., move intermittently, and handle emotional or physically demanding situations.

Work Environment

  • Works in various healthcare settings (nurses' stations, patient rooms, administrative offices).

General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by
your supervisors.

Essential Duties

Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.

Use only authorized abbreviations established by this facility when recording information.

Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.

Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.

Report all accidents and incidents you observe on the shift that they occur.

Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.

Report any known or suspected unauthorized attempt to access facility's information system.

Perform only those nursing care procedures that you have been trained to do.

Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).

Greet residents and escort them to their room.

Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.

Make resident comfortable.

Inventory and mark the resident's personal possessions as instructed.

Store resident's clothing.

Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.

Transport residents to new rooms or to the receiving area.

• Assist with loading/unloading residents' to/from vehicles as necessary.

• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.

• Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.

• Report all complaints and grievances made by the resident.

• Participate in and receive the nursing report as instructed.

• Follow established policies concerning exposure to blood/body fluids.

• Make beds as instructed.

• Put extra covers on beds as requested.

• Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed.

• Ensure that residents who are unable to call for help are checked frequently.

• Answer resident calls promptly.

• Check residents routinely to ensure that their personal care needs are being met.

• Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).

• Record the resident's food/fluid intake. Report changes in the resident's eating habits to the Nurse Supervisor/Charge Nurse.

• Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident.

• Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.).

• Participate in and receive the nursing report upon reporting for duty.

• Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.

• Follow work assignments, and/or work schedules in completing and performing your assigned tasks.

• Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.

• Notify the facility when you will be late or absent from work.

• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.

• Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary).

• Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled).

• Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.).

• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

• Perform restorative and rehabilitative procedures as instructed.

• Assist in preparing the resident for a physical examination.

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.

• Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).

• Report injuries of an unknown source, including skin tears.

• Observe disoriented and comatose residents. Record and report data as instructed

• Perform special treatments as instructed.

• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.

• Use only the equipment you have been trained to use.

• Report defective equipment to the Nurse Supervisor/Charge Nurse

• Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

• Report all allegations of resident abuse and/or misappropriation of resident property.

• Honor the resident's refusal of treatment request. Report such requests to your supervisor.

Qualification

  • Education and/or Experience
  • Must possess, as a minimum, an 10th grade education

Language Skills

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and
  • procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.

Mathematical Skills

  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations

  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.

Assist in organizing, planning and directing the medical records department in accordance with established policies and procedures. Assist the Medical Records/Health Information Consultant as required. Maintain minutes of meetings. File as necessary. Develop and maintain a good working rapport with inter department personnel, as well as other departments within the facility, to assure that medical records can be properly maintained. Assist in recording all incidents/accidents. File in accordance with established policies and procedures. Retrieve resident records (manually/electronically). Deliver as necessary. Files information such as nurses' notes, resident assessments, progress notes, laboratory reports, x ray results, correspondence, etc., into resident charts. Collect, assemble, check and file resident charts as required. Assist MDS Coordinator in scheduling assessments in accordance with current facility and OBRA guidelines. Ensure incomplete records/charts are returned to appropriate departments or personnel for correction. Assist in developing procedures to ensure resident records are properly completed, assembled, coded, signed, indexed, etc., before filing. Establish a procedure to ensure resident charts/records do not leave the medical records room except as authorized in our policies and procedures. Maintain a record of authorized information released from charts/records, i.e., type information, name of recipient, date, department, etc. Abstract information from records as authorized/required for insurance companies, Medicare, Medicaid, VA, etc. in accordance with current Privacy Rules. Index medical records as directed by the medical records/health information consultant. Maintain various registries as directed including register for admission and discharge of residents. Transcribe and type reports for physicians as necessary. Collect charts, assemble them in proper order, and inspect them for completion. Pick up and deliver resident medical records from wards, nurses' stations, and other designated areas as necessary. Batch resident information into the computer and retrieve resident demographic information as appropriate or as instructed. Answer telephone inquiries concerning medical records functions. Prepare written correspondence as necessary. Retrieve medical records when requested by authorized personnel (i.e., physicians, nurses, government agencies and personnel, etc.) Assure that medical records taken from the department are signed out and signed in upon return to the department. File active and inactive records in accordance with established policies. Index medical records as directed. Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. Report any known or suspected unauthorized attempt to access facility's information system. Assume the administrative authority, responsibility, and accountability of performing the assigned duties of this position. Committee Functions Perform secretarial duties for committees of the facility as directed. Collect and assemble/compile records for committee review, as requested, and prepare reports for staff/other committees as directed. Personnel Functions Report known or suspected incidents of fraud to the Administrator. Ensure that departmental computer workstations left unattended are properly logged off or the password protected automatic screen saver activates within established facility policy guidelines. Staff Development Attend and participate in mandatory facility in service training programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention, etc.). Attend and participate in workshops, seminars, etc., as approved. Safety and Sanitation Report all unsafe/hazardous conditions, defective equipment, etc., to your supervisor immediately. Equipment and Supply Functions Report equipment malfunctions or breakdowns to your supervisor as soon as possible. Ensure supplies have been replenished in work areas as necessary. Assure that work/assignment areas are clean and records, files, etc., are properly stored before leaving such areas on breaks, end of workday, etc. Budget and Planning Functions Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc. Other duties as assigned Supervisory Requirements ou are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Qualification Education and/or Experience Must possess, as a minimum, a high school diploma or GED. Must be able to type a minimum of 45 words per minute and use dictation equipment. A working knowledge of medical terminology, anatomy and physiology, legal aspects of health information, coding, indexing, etc., preferred but not required. On the job training provided in medical record and health information system procedures. Must be knowledgeable of medical terminology. Be knowledgeable in computers, data retrieval, input and output functions, etc. Language Skills Must be able to read, write, speak, and understand the English language.
