Job Description:

Position Summary:

This role is responsible for the efficient, ethical and lawful compounding and dispensing of prescription pharmaceuticals. Advises customers on the use and storage of prescription and non prescription drugs. Orders and maintains supply of pharmaceutical stock. Oversee pharmacy technicians as needed. 

Here's what you'll do:  

  • Receive prescription and refill requests from customers and/or clinics and fill prescriptions requests efficiently and accurately and in a professional and courteous manner.  

  • Dispense medications as prescribed; oversee and ensure accurate retrieving, counting, pouring, weighing, mixing, packaging and/or labeling of products.  

  • Provide medication counsel and advise customers on both prescription and over-the-counter medications regarding use, dosage, drug interactions/side effects, storage, etc. 

  • Responsible to order, stock, check inventory, and perform cycle counts in the pharmacy, for both prescription and over-the-counter items and maintain familiarity of department products.  

  • Review and validate received orders for amount, quality, count or weight, condition and ensure all products are stored in appropriate areas.  

  • Ensure all out dated/damaged products are handled per established guidelines.  

  • Maintain Pharmacy records to include, but not limited to, customer profiles, insurance forms, pharmacy record/POS systems, inventories, control records and registries (i.e., poisons, narcotics, controlled substances, etc.)  

  • Lead and/or oversee pharmacy technicians and/or pharmacy interns as needed to ensure efficient and accurate service is provided.  

  • Communicate with the Pharmacy Manager about any potential concerns.  

  • Maintain a clean and well-stocked department; ensure the department complies with Company standards.  

  • Maintain and implement pharmacy programs (i.e. security measures, immunizations and other clinical programs) accordingly.  

  • May be responsible to provide clinical care services as assigned.  

  • Maintain current knowledge of legal regulations, industry trends and best Pharmacy practices; make recommendations to improve current programs and processes and to ensure company legal compliance/ Comply with all applicable federal and state regulations.  

  • Understand and communicate Company service and loyalty programs and model high standards of service to achieve a customer oriented store.  

  • Additional responsibilities may be assigned as needed. 

Here's what you'll need:   

  • Bachelor's Degree (Required) in Pharmacy or related field required. Doctorate degree in Pharmacy preferred. 

  • Three years experience in pharmaceutical department. 

  • Pharmacy License (in good standing) required in the state of operation.  

  • Immunization Certification preferred. 

  • Strong knowledge of HIPAA Privacy and Security Rules, pharmacy laws, rules, and guidelines.  

  • Strong written and verbal communication skills; strong customer service skills, Strong multi-tasking, organization, attention to detail and problem-solving skills.  

  • Good leadership abilities with capability to work in a hands-on environment.  

  • Successfully completes required continuing education hours and maintains Pharmacy License in good standing.  

  • Working knowledge of perpetual inventory procedures.

**Responsibilities**
Mayo Clinic Pharmacy Services and Mayo Clinic Primary Care is seeking an Ambulatory Care Clinical Pharmacist to join our team in Menomonie, Bloomer, and Mondovi, Wisconsin for a primary care ambulatory care pharmacist position. This is a 0.8 full time equivalent (FTE) position. Hours are 8:00 a.m. to 5:00 p.m., on 4 days per week (excluding clinic holidays) for the days worked.
The individual in this role will be responsible for covering primary care family medicine clinic at the Menomonie campus 3 days per week as well as a half day coverage per week for 2 family medicine clinics in Bloomer and Mondovi. Responsibilities include Medication Management Services (MMS) including comprehensive medication reviews, targeted medication reviews, and non-visit care. As part of this role, the pharmacist will be responsible for patient and provider questions, team meetings, quality improvement, and practice management and expansion in addition to patient care activities.  There is potential for research and precepting in addition to the patient care activities. There may be opportunity for remote work on occasion for patient video visits and phone calls.
The pharmacist will work with multidisciplinary and collaborative team members in the primary care outpatient clinic. Those team members include nurses, physicians, advanced practice providers, care managers, and additional support personnel to evaluate, plan and monitor therapy across the continuum of care. The MMS pharmacist will design, implement, monitor, evaluate, and modify patient pharmacotherapy to ensure that medication therapy is indicated, effective, safe, and convenient and will work with a collaborative practice agreement. The pharmacist will participate in relevant committees.
**Qualifications**
**Required Qualifications**
+ Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
**Additional Qualifications**

+ Current pharmacist license by the Wisconsin Board of Pharmacy, or eligible to obtain within a time frame compliant with the site's applicable employee licensure policy (60-days after start date). Scope of Position

The Medication Management / Transitions of Care (TOC) Pharmacy Technician provides a valuable patient-centered service by focusing on facilitating TOC, including obtaining and documenting a complete and accurate admission medication history and allergy information in the electronic medical record (EMR) as well as assisting with the discharge medication process. The TOC Pharmacy Technician completes the admission medication history during an inpatient admission, ambulatory encounter, pre-admission testing, surgery encounter, or within a procedural area under the supervision of a pharmacist, physician, or advance practice provider. Information can be gathered from a face-to-face or telehealth interview with the patient chart and communication with patients, caregivers, physicians, retail pharmacies, and long-term care facilities. The TOC Pharmacy Technician will make a good faith effort to ensure the documented information is complete and accurate by identifying and facilitating correction of medication discrepancies or escalating to a pharmacist or provider for follow up, if needed, thereby aiding in the hospital's medication reconciliation efforts. Throughout the hospital admission, the TOC Pharmacy Technician is responsible for collaborating with the patient and care team to facilitate discharge, which includes communicating options for discharge medications and long-term medication delivery with the patient and/or caregivers, obtaining credit card information and facilitating Meds to Beds medication delivery, if applicable, and assisting with accessing coverage for medication, and engaging the appropriate staff such as the Medication Assistance Program if medication access needs are identified or case management if a prior authorization is required.

The TOC Pharmacy Technician promotes a culture of safety by adhering to all hospital and department rules, regulations and requirements including those outlined in the Medication List Documentation and Reconciliation policy, education videos, and Standard Operating Procedures. The TOC Pharmacy Technician must participate in continuing educational opportunities to meet the annual competency requirements and participate in continual training opportunities provided by the department to develop ongoing skills in other areas of pharmacy. The TOC Pharmacy Technician acts as a positive role model for all TOC pharmacy technicians and acts in an encouraging manner conducive to the hospital, department and administration's visions and values.

Position Summary

The TOC Pharmacy Technician is responsible for obtaining complete and accurate admission or ambulatory medication history and allergy information. The TOC Pharmacy Technician will help to streamline and facilitate the discharge medication delivery process when applicable.
