The Nurse Practice Manager maintains full accountability for leading and managing the human resources, clinical, financial and administrative operations of an assigned practice(s) in order to continuously improve the quality of patient care and create a healthy workplace for staff in collaboration with organizational leadership.
**Responsibilities:**
+ Determine and flex staffing standards based on patient care needs, patient flow, medical acuity, procedures, staff skills and other clinical needs. + The Nurse Manager is administratively accountable for the management of all activity within the practice. Demonstrates the ability to create a shared vision that applies to critical thinking skills and utilizes financial and quality data and conceptual knowledge in the development of the department vision and operational plan. Orchestrates complex change and acknowledges the psychological transition on self and others. Involves stakeholders and experts in planning, designing and redesigning change. Uses appropriate interpersonal styles and techniques to gain acceptance of ideas or plans; modifying one's own behavior to accommodate tasks, situations, and individuals involved. Supports staff during times of difficult transitions.
+ Analytical ability necessary to evaluate staff, address and improve clinical issues and devise solutions to complex problems. Leadership ability to develop staff, establish and maintain standards of performance. Superior interpersonal and communication skills sufficient to effectively create relationships to support a healing environment with patients, families, staff, physicians, and administrative personnel including senior management in both written and verbal communication. Ability to work productively and effectively within a complex environment.
+ Demonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care. Provides oversight and supervision to the overall practice environment, assisting all other disciplines and directing clinical/administrative activity. Oversees daily operations of practice with regards to patients, staff, physicians and ancillary support/departments. Assumes formal leadership responsibilities in the following: budget and financial formulation, staff levels and coverage, human resource management, departmental development and education and safety/regulatory reporting and preparation. Serves as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership. + Collaborates with medical staff to monitor patient outcomes; set clinical quality priorities, recommend and implement professional improvements and initiatives and implement practice-based safety initiatives. Supports and encourages involvement of staff in the development and implementation of evidence-based practice and quality improvement initiatives. Actively oversees all aspects of regulatory compliance, including all outside agencies and audits as well as internal requirements, such as core competencies and training. Utilizes research and evidence-based practice to support improvement in clinical care: identifies research issues or articles related to clinical specialty or areas of interest.
+ Sets high standards of performance for self and others by imposing standards of excellence and development of inquiry skills that are innovative, optimistic and supportive of professional growth. Creates developmental paths and plans for staff to increase the expertise and caliber of staff. Initiates innovation in staff development, clinical orientation, continuing education and supports specialty certification. Responsible for developing, marketing and coordinating programs to provide education and enhance consumer awareness. Oversees and manages the interviewing, selection, hiring, training, evaluating, and progressive discipline process of staff. Supports the development of students, new staff and colleagues. Plans for orientation, training, preceptor initiation and evaluation period of new staff in department. Creates an environment of open dialogue, inquiry and continuous development by asking for feedback and improving practice.
+ Develops and maintains productive working relationships internally and externally by building teams and relationships through mentoring and modeling uplifting and positive communication. Applies national best practices and uses evidence based practice to build a culture of excellence in patient care. Sets clinical, financial and human resources priorities for improvement in concert with organizational goals. Demonstrates sound fiscal responsibility in the development and management of the practice budget and holds staff accountable for the efficient use of resources, as well as optimization of billing and documentation practices. Oversees program and patient coordination activities to ensure program goals and objectives are achieved by providing direction to staff, maintaining efficient operations and overseeing quality of patient care provided. Develop and optimize the clinical care tools of the practice and the physicians (scheduling templates, space, staffing mix and patient care protocols) that improve the quality of the overall product delivered to the patient.
+ Minimum 5 years Nursing Experience required.
+ Progressive leadership experience with relevant clinical experience preferred.
+ BSN or Bachelors in related health care field required. MSN or Masters in a health care field preferred.
+ The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.

+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

Position Summary
Physicians provide professional comprehensive medical services for NeighborHealth Center (NHC) patients on a full-time basis as described in their employment agreement. As a member of a Care Team, the Family Medicine Physician with Prenatal Care works with the other care team members to provide quality patient-centered care.
Principal Duties and Responsibilities*
- Provides comprehensive primary care services to patients of all ages
- Offers routine family-centered prenatal care through delivery, with referral to OB partners for delivery; conducts postpartum follow-ups.
- Oversees and delivers NHC’s women’s health clinical services, collaborating with Advanced Practice Providers.
- Performs basic primary care office procedures, including women’s health procedures such as colposcopy and LARC (IUD, Nexplanon).
- Obtains health history and performs a physical exam to determine and execute a diagnosis and treatment plan in accordance with established standards, procedures, and practices. Requisitions lab tests and x-rays, writes prescriptions, and makes referrals as appropriate. Follows NHC clinical practice guidelines.
- Participates in the call schedule on a predetermined basis.
- Delegates patient care responsibilities that fall under the scope of practice of other Care Team members to those individuals (medical assistant, patient educator, registration representative, behavioral health).
- Attends staff and medical provider meetings and maintains a level of involvement with the health center committees, as needed, to ensure quality patient care. Participates in peer/chart review, performance evaluations, and quality improvement efforts.
- Provides patient care to approximately 9-10 patients per 4-hour shift per day as per productivity expectations for all providers.
- Collaborates with nurse practitioners or supervises physician assistants, as appropriate, in providing patient care; may assist in the education training of medical students and residents.
- Participates in continuing medical education, including Life Support Training, to maintain the skills necessary to perform physician duties and as required by applicable medical boards and the State.
- Maintains own physical health required to perform physician duties; maintains current immunizations and up-to-date PPD.
- Maintains communication with administration regarding personal issues related to drugs, felonies, discipline, loss of license or privileges, litigation where the provider or clinic is harmed or at risk, or other serious issues.
- Participates in quality improvement efforts on the Care Team.
- May perform other duties as assigned by the Chief Medical Officer.
Compliance and Documentation
- Confirms patient identity according to current procedure before rendering any service.
- Thoroughly, accurately, and promptly documents all services rendered in the electronic medical record for every patient.
- Performs all tests, procedures, and other services according to the policy and procedure of NHC, notify the appropriate supervisor in a timely manner of instrument or procedural problems.
- Is knowledgeable of and in compliance with CLIA, OSHA, HRSA, and JCAHO regulations for safety, infection control, equipment operation, and other applicable areas.
- Participates in proficiency testing as required by NHC or departmental policy to ensure that proper standards of care are being maintained
- Maintains confidentiality of patient health information in accordance with HIPAA and other applicable regulations.
Required Skills or Abilities*
- Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel with keyboarding skills of at least 45 wpm.
- Ability to cultivate and develop inclusive and equitable working relationships with co-workers and community members.
- Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.
Required Knowledge, Experience, or Licensure/Registration
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
- Completion of residency program with board certification (or eligibility) in Family Medicine, and training and clinical experience sufficient to deliver quality prenatal care and office-based women’s health care (OB fellowship not required).
- Board Certification (or eligible) in specialty training and maintenance of certification
- State licensure to practice medicine in North Carolina

- Ability to work onsite/hybrid/remotely Mondays through Fridays during the hours of 8am and 5pm.

The Care Manager will work standard hours, Monday through Friday, in a flexible, hybrid work structure! This role will work with Erie HealthChoices members, supporting member needs remotely and out in the community. The Care Manager may be on-site at provider locations up to 50% of the time, while the remainder of work responsibilities can be done at home. This role also has the opportunity to utilize office space at the 380 E Bayfront Pkwy location, when needed, for convenience!
The Care Manager is responsible for assisting members identified as at-risk for recidivism, discontinuous care, or as members of priority or special needs populations who present with complex needs for coordination of their behavioral health services with other aspects of their care. This role will work specifically with the prenatal and postnatal member population within the framework of the Community Based Care Management Initiative!
This specialized role is responsible for assisting assigned and specific members with care at all levels of the continuum, and for providing any and all required pre-certification, continued stay and/or discharge reviews, service authorization, and care coordination, as needed. The Care Manager will focus on the needs of our adult members that are pregnant or postnatal, for up to one year after delivery, providing support and connecting them to resources.
Care Managers represent the organization to providers, member groups, and families, and participate in the overall administration of clinical operations as warranted. This role is expected to bring a level of clinical leadership to the care management department, and to execute their responsibilities consistent with the applicable Community Care Policies and Procedures. Care managers are specifically chosen based on a targeted area of practice, supported by education, training, and experience, with expertise in the delivery of behavioral health care to a given population. In addition, the Care Manager may serve as the care management lead for other members of his/her team
**Responsibilities:**
+ Makes authorization determinations for medically necessary services independently, within the scope of the practice of held licensure.
+ Demonstrates knowledge of clinical treatment, case management and community resources.
+ Encourages coordination of care with primary care physician and other service providers integral to the member's life.
+ Monitors and evaluates effectiveness and outcome of treatment and service plans and recommends, modifications as necessary to provide optimal clinically appropriate services with a goal of maintenance in the community at the least restrictive level of care.

+ Maintains contact with and refers members to community based case management services as appropriate.

+ Pennsylvania Licensure in health or human services field and master's degree OR licensed RN (BSN preferred) OR Licensed Behavior Specialist (For IBHS levels of care only). Preference will be given to RN, LSW, LCSW, LPC, licensed MFT, and/or a licensed PhD (psychologist)
+ Minimum of three years of relevant clinical experience.
+ Experience in working with a prenatal/postnatal population is a bonus.
+ Experience in managed care strongly preferred.
+ General knowledge of best practices in behavioral health, emphasizing work with special needs or priority populations and in public sector systems.
+ Certification as a Certified Addiction Counselor (CAC) or Certified Alcohol and Drug Counselor (CADC) is helpful.
