Work with Practice Managers to direct and manage the center care team

Required Qualifications:

- Acts as the primary care physician for PACE participants in community and institutional settings.

- Performs initial and semiannual evaluations.
- Establishes collaborative practice with PACE nurse practitioner.
- In conjunction with nurse practitioner, performs routine. assessments and examinations of PACE participants.
- Together with the interdisciplinary team, develops care plans for PACE participants.
- Works collaboratively with members of interdisciplinary team.
- Communicates with consulting physicians and integrates recommendations into plans of care.
- Prescribes in accordance with state and federal regulations.
